  2. Flume pipe at Cwm, Blaenau Gwent An Off Track team were working on a flume drain at Cwm in Blaenau Gwent Part of the task was to release an obstruction from inside the pipe. To stop the flow of water whilst the work was carried out, the Injured Person (IP) and Team Leader (TL) had walked up the bank to divert water away from the top of the flume 2

RIDDOR Accident Cwm (WVL) 12/01/14 • Once complete, the IP and TL made their way to bottom of the bank • Whilst doing so, the IP slipped on the wet underfoot conditions and fell • In holding out his arm to break the fall, it resulted in him dislocated his right shoulder • Paramedics were called who administered immediate treatment to the IP which was effective enough to enable them to walk the IP off site and convey him to hospital for further treatment

Cwm (Western Valley Line) 7m 60ch • On arrival at hospital the Injured Party had his arm relocated into his shoulder • After a home visit it has been established that he is in a comfortable condition, although his arm is in a sling • It is expected that the IP will be absent for a number of days

Accident site • The gradient of the bank was estimated to be 25 degrees • What you need to do • Clear the site of hazardous conditions and obstructions that may affect your working conditions as far as practicable before commencing work • Consult Task Risk Control Sheets NR/L3/MTC/RCS0216/GH02 Underfoot Conditions • Be aware of wet, frosty and icy conditions - particularly at this time of the year IP slipped here 5
