  2. THT Members The Trenton Health Team (THT) is a non-profit corporation. Currently the team is made up of two people from each of the Following institutions: • Henry J. Austin Health Center Inc. • Capital Health Inc. • St. Francis Medical Center • City of Trenton Department of Health and Human Services

  3. THT Origins • Study of the health care needs in the city • Discovered high utilization of the city’s emergency rooms by the residents of Trenton • The need for Specialty Care Services • Lack of communication between the health care providers in the city

  4. THT Vision and AIM To measure the health care gaps in the city of Trenton and develop innovative solutions to fix the gaps identified, so that providers can give quality health care at a reduced cost to EVERY resident in Trenton.

  5. THT Governance Phase I • Board members include 2 representatives from the four health care institutions • The board members meet once a week • Bylaws have been established Phase II • Establish an advisory committee (community partners), to include key community stakeholders, like city residents, consumer groups, nursing associations and mental health providers.

  6. THT Community Involvement • Working with the City to create a Community Health Improvement Plan. • Setting up the Community Advisory Board.  • Working on understanding the numerous factors (e.g., sociodemographic, community, societal, personal) influencing the health promoting behaviors in our community (National Institute of Health)

  7. THT Current Community Projects • Building a Trenton Health Information Exchange (HIE) • Community Collaboration around Emergency Room visits • Partnership between HJA and the mental/behavioral health partners in the city

THT Finance Working on the budget and sustaining our efforts: • Received $1.5 million over 5 years to assist with starting up a Health Information Exchange system in Trenton. • THT is working on other grants to support ongoing costs. • THT is considering the role of an Accountable Care Organization.

  9. What is an Accountable Care Organization? An Accountable Care Organization (ACO): • Integrated health care system • Primary care physicians, specialists and hospitals work together to manage the overall care of their patients • This done in such way that there is substantial cost savings • Savings are then shared amongst the partners

  10. ACOs continued . . . . The ACO concept is a critical piece of the government’s current health reform agenda.  ACOs aim to create a health system focused on coordinated care and clinical best practices by holding all members of a patient’s care team jointly responsible for the quality and cost of care.

  11. How do we get ready for ACO reimbursement? We must build an infrastructure that includes: • Health Information Exchange • Improvement in key quality health outcomes • Improve our inefficient health care workflow and processes • Demonstrate that we can reduce health care cost • Collaborate with area health care providers and institutions (this is a requirement to be a part of an ACO)
