  2. Purpose Statement We will be able to use exit slips to evaluate our teachingin order to improve student learningby creating and evaluating short daily formative assessment based on standards.

  3. What are Exit Slips? • Formative Assessments • Used to measure the daily purpose • Should match the task for the lesson • Should match the skill for the lesson • Used to drive instruction • Exit slips need to be aligned to the purpose statement, skill and task • Aligned to the standards

  4. What Exit Slips are NOT • Students writing what they learned from the lesson, their opinion or things they want to learn.

  5. Exit Slip Ideas • Graphic Organizer • Multiple Choice • Multiple Choice + Explanation • Writing/explanations • Word Problems

  6. Primary ELA Example We will be able to describe how an illustration supports the text in order to determine what the author wants us to know by locating details within the illustration.

  7. Elementary ELA Exit Slip Example We will be able to determine the author’s opinion of an informational text in order to understand the author’s viewpoint on a topic by using a graphic organizer.

  8. Middle School Exit Slip Example We will be able to determine the central idea of an informational text in order to understand what the text is mainly about by writing a summary and supporting details.

  9. Primary Math Exit Slip Example We will be able to compare two numbers between 1 and 10 presented as written numerals in order to understand which number is greater by using manipulatives.

  10. Elementary Math Exit Slip Example We will be able to explain and justify the placement of a number on a number line in order to understand the position of a number by justifying our thinking.

  11. Math Exit Slip Example We will be able to write a decimals and fractions in equivalent forms in order to understand how decimals and fractions are the same by using conversion methods.

  12. What Do I Do With the Exit Slips? At the end of each lesson, check each exit slip and record the information on the progress monitoring tool. The progress monitoring tool should be used everyday for every objective in each content area.

  13. Insert Measurable purpose statement with what, why and how. Insert the standard that aligns to your lesson’s purpose. Record the information and determine whether or not to reteach or identify students who need intervention. Insert student's names and mark if they mastered the skill.
