Participation in evidence-based self management programs improve health status, health behaviors and healthcare utilization Target population: the aging populations suffering from multiple chronic diseases • SM interventions improve the health experience of individuals managing multiple chronic conditions through: • Symptoms and disease management • Emotional well being • Skills for daily living • Examples of SM interventions include: • Chronic Disease Self-Management Program • Diabetes Self-Management Program • Arthritis Self-Management Program Outcomes • Illustrative outcomes achieved: • Improvements in psychological health status • Improvement in self-efficacy • Improvement is selected health behaviors • Net savings $390-$520 per participant over a 2-year period • Examples of SM program quality indicators: • Number of individuals that rate high confidence in managing a chronic condition on a daily basis • Number of systems that incorporate SM support as part of their quality improvement plan Source: 1.
