  3. Suggested Supplies Supplies Needed for Class Daily: • Always, always, always bring the following to class: • 1. Math Composition book • 2. Loose-leaf notebook paper (assignments on spiral paper, torn out of the notebook, will not be accepted.) • 3. One highlighter, any color • 4. At least two sharpened pencils • 5. Blue, Green, or Red ink pens • 6. Math workbook • 7. Index Cards (3 X 5) • 8. Scissors • 9. A pocket section for Math in a 3-ring binder • 10. Tape • 11. Glue Stick Supplies Requested for Use by Class: • Please provide your teacher with these items so that important documents and notes can always be available and the classroom may be maintained and sanitary regardless of the paper shortage and budget cuts: bring the following optional items to class (students will be rewarded with Perdomo Points which can be used to purchase prizes): • 1. Copier paper • 2. Tissue • 3. Hand Sanitizer • 4. Clorox wipes • 5. Paper Towel • 6. Science boards • 7. Individually wrapped candy/treats • 8. Post It Notes

Textbooks • Textbooks are to remain in the classroom desk at all times. • Work texts/workbooks are to be kept in the student’s book bag and brought to class and home daily. • Online textbook can be accessed through the link on the student portal or directly through: http://my.hrw.com • Username: seahawk2 • Password: 12345

  6. Classroom Rules “R” 1. Respect yourself, your fellow students, and most of all, your teacher. 2. No eating, drinking or chewing gum in class. Water bottles permitted. 3. No talking out of turn. 4. Avoid unnecessary conversation. 5. Respect school property. 6. Take your seat and begin the warm-up immediately upon entering the room. 7. Stay in your assigned seats unless you have permission to stand. 8. Always come prepared for class. 9. Any items not used exclusively for school purposes should be left at home or kept out of sight, this includes cell phones which must be turned off. • Class ends only when I have dismissed you. • Be honest

  7. Uniform Policy • Middle School uniform consists of a navy blue shirt with the school logo. • Boys: Khaki pants or shorts with a belt. • Girls: Khaki shorts or skirt, (no shorter than finger tips) or khaki pants. • Jackets: Jackets are permitted as long as official uniform is underneath.

  8. Progressive Discipline Plan If any student does not adhere to the above Rules and Expectations, the following steps will be taken. Depending on the severity of the situation, steps may be skipped.  • Warning – A student will be verbally warned when a disruptive behavior is exhibited. • Conduct Cut and note in Grade Book – students conduct grade will be lowered by one letter grade for the marking period. A student may recover one grade per marking period based on observation of corrective behavior. • Detention – student will be assigned a 30-60 minute detention to be served before or after school, or during lunch time. • Parent Contact – I will notify the parent via email, telephone call or message in the organizer explaining the situation. • Student/Teacher/Parent Conference – When a situation requires the meeting of student and parent/guardian, a team conference will be scheduled to discuss the student’s conduct and/or academic progress. Strategies will be developed to assist the student and parent with the situation. • Administrative Referral – Continuous or serious situations will result in a written referral to the grade level administrator and administrative consequences.

  9. Attendance/Tardies • If your child is absent from school, he/she will have up to 3 days to bring in a note for the absence to be excused. It is your child’s responsibility to obtain their missed assignments. This must be done outside of class time (7:45 a.m.) • Tardies: School begins at 8:35 a.m. Students arriving at 8:36 a.m. will be marked tardy. Students arriving after 8:45 a.m. must go to Student Services to obtain a tardy admit to class. • ABSENCES SERIOUSLY AFFECT YOUR GRADES AND DISTURB THE LEARNING ENVIRONMENT!!! Please do not schedule vacation on school days.

  10. Criteria for Fieldtrips • Fieldtrips are REWARDS in Middle School!!! • Students must maintain an “A, B, or C” average in conduct to be eligible for fieldtrips. • Students must not have more than 3 unexcused absences or tardies per grading period in order to participate.

  11. Home Learning Home Learning is assigned EVERY NIGHT. It is the students’ responsibility to copy their homework assignment from board. Please make sure you check your child’s agenda as it also makes a great way to communicate. HOMEWORK POLICY Students who do not complete their homework will be sent to another class to complete their assignment and miss valuable instruction. Every assignment given, those in class and at home are imperative to the learning process. Students may also be mandated a “Homework Detention” to be served during the week from 3:05-4:05 p.m. during which time the assignment will be made up/completed.

  12. Point Values • In order for your child to pass each class he/she must obtain 10 points in each class for the year. • Below is the breakdown of points for quarter grades: A=4 x 2.5 B=3 x 2.5 C=2 x 2.5 D=1 x 2.5 F=0 x 2.5

  13. Conferences • Appointments for conferences can be made through your child’s teachers. • Conference times are usually in the morning, but may vary depending on the day and/or the teacher. • Thank you for understanding.
