Cas 2: Patient de 20 ans présentant: -Une douleur faciale et dentaire avec fébricule depuis 72h malgré le traitement par Augmentin. -Un trismus, dysphagie, hypersialorrhée. -Tuméfaction faciale avec extension submentale et cervicale. ATCDs de traitement dentaire des 23-24èmes dents il y a 1 mois, pas d’angine récente. 1/ Diagnostic? 2/ Examens cliniques et radiologiques à faire en complément? 3/ CAT?

  3. Réponses 1/ Cellulite cervico-faciale d’origine dentaire. 2/ Examen stomatologique Radio panoramique, dentascan. Maisavant: TDM cervico-faciale et thoracique (extension locorégionale; médiastinite?) 3/ Selon le cas: • Traitement antibiotique par voie générale (probabiliste puis ajustement selon antibiogramme si disponible) • Mise à plat chirurgicale + ttt antibiotique (IV)

  4. TDM avec injection, coupe passant par la mandibule. TDM avec injection passant par la région sous –hyoïdienne.
