  3. Targeted Preventative services include: • Housing related floating support • Health and wellbeing • Healthy eating • Exercise • Arts Culture • Volunteering, befriending and linking with communities • Linking with GP surgeries

  4. Who services are for • People on the cusp of needing care packages • All client groups • People who are vulnerable because of e.g.: • Social isolation • On the verge of a crisis • Not able to manage their tenancy • Frailty

  5. Outcomes • Increasing independence • Maintaining levels of independence • Preventing or delaying need for long term care • Helping people step down from long term care • Helping people after reablement, enablement, recovery • Moving through services towards use of community based services

  6. Services elementsfloating support • Redesigned housing related floating support • Neighbourhood based as now • All client groups except specialist MH • Enable people to move to other support services when needs reduce • Contract volumes reduced

  7. Housing related floating support outcomes • Ability to manage home • Skills based • Ability to manage finances • Ability to manage self care • Skills based and confidence building • Maintain quality of life Services to take an enablement focus

  8. Health and Wellbeingall groups for 18 years+ Aiming for: • Innovative programme of activities to include: • healthy eating, exercise, art and cultural • Programme – not stipulated – Council welcome proposals • Bids to be based and evaluated on a best value approach • Service to start Autumn 2013 • Want activities in a range of venues across Hackney • Enable people to move to other community services when needs reduce

Healthy Eating • This element would include: • Nutritional advice • Hints and practical sessions on sowing/growing herbs or vegetables in window boxes or pots • Identification of range of vegetables now available at markets/supermarkets, where they come from, how to prepare and cook • Menu suggestions for preparation of simple healthy meals for one • Best practice with regards to sell-by dates, using frozen meals, freezing meals and use of micro-waves

  10. Exercise and arts activities • Expecting bidders to design activities to meet a diverse range of needs • Aiming for a range of different types of exercise such as dance, walking groups, Tai Chi, yoga, chair based exercise and suited to those with poor mobility • Aiming for a range of different types of arts and cultural activities, such as, discussion groups, painting, drawing, using IT as an art technique, history, including local history

  11. Volunteering and BefriendingService starts September 2013 Ambitious aims: Volunteers to • provide home visits and support to people to enable them to link to local community and activities • develop self-help/peer support groups • undertake telephone checks • give practical support in peoples’ homes, gardens and community • Volunteering to improve self confidence, feelings of worth and support with a pathway to employment • Service could be delivered through elements of timebanking • Service may be subject to TUPE re some LBH staff

Configuration of contractsto be confirmed Options being considered: • Neighbourhood floating support • 4 lots/areas • Health and wellbeing service - one lot • One borough wide service • Could include sub contracting/partnering/SPV • Volunteering and befriending - one lot • One borough wide service Providers might bid for 2 lots

Provisional Timescales Aiming for: • Cabinet procurement committee for permission to tender Feb 2013 • Pre qualification questionnaire PQQ late Feb • Invitation to tender ITT April • Contract start HWB, B&V autumn 2013 • Contract phased start floating support Dec 2013 but transition plan from Sept 2013
