  4. Assisted Living Facilities These facilities provide assistance for seniors who need help maintaining activities of daily living (ADL’s). Bathing Dressing Food Preparation Medication Assistance Transferring Memory Care Issues Long Term Care Insurance Policy

  5. Assisted Living Facilities Studio or one bedroom apartments Common dining area Three meals a day Assistance with ADL’s Person needs to be able to leave the facility with one person assist Private pay New Choices Waiver Program

  6. Skilled and Intermediate Care Facilitiesaka: Nursing Homes These facilities provide assistance for seniors who need help with all ADLS’s or have considerate cognitive issues. Based on medical model Usually semi-private rooms All meals prepared Recreational and social work activities provided All medication distributed by nursing staff Hands on assistance with bathing, dressing, eating, transferring Followed closely by the attending physician

What to Look For in a Long Term Care Facility Always visit the facility before admission • Interview staff • Talk to residents and families who live there • Contact your local Ombudsman Program • Check annual surveys and Medicare.gov • Make basic observations • Is there any odor? • Are the residents clean? • Is the facility clean? • Are residents engaged in activities? • Are residents restrained or sedated? • Are call lights unanswered? • Can you observe adequate staff? • Does the physical environment look safe?
