  2. AIM (Plan) • Engage/retain adolescents in treatment (post residential placement) and promote a recovery process in home environment • Make referrals from START to home area treatment provider • Support recovery process in home area

  3. Change (Do) • Enlighten participants that life changes need support after being in residential • START staff make specific appointment for outpatient treatment/advocacy help with mentors • Family support group participation encouraged • Outpatient providers offer groups for treatment and family support

  4. RESULTS (study) • Referrals from START residential to outpatient treatment were minimal • All START discharges have outpatient referrals • Referrals to outpatient treatment at RWJ grant onset in 2008 were nonexistent • Referrals to all five outpatient counties February 2008 -- February 2009 • Fayette County 5; admitted 3 • Lamar County 3; admitted 3 • Marion County 8; admitted 5 • Walker County 6; admitted 3 • Winston County 1; admitted 1

  5. NEXT STEPS (Act) • Year 2 is continuing with engagement, adding continuing care and wrap around • Offering in-home services • Making linkage referrals • Following up on residential referrals more closely with case management • Following up on residential case management referrals and making any new identified linkages using case management

  6. IMPACT (Business Case, Lessons Learned) • Networking, increased communication, specific appointments arranged, follow up with missed appointments. . .equals increased engagement in treatment from residential to outpatient services • Meeting the client where they are and moving forward fosters desire to participate • In-home services adds the family component that has been minimal or missing.
