Your Services Are Changing Let’s Talk • This Strategy is our plan for improving the lives of people with a learning disability and their carers who live in Warwickshire. • The 3 year plan tells you how services will work to make sure people with a learning disability have the same chances and choices as everyone else.

  3. Our Vision • We will change how we do things by making sure we have a person centred approach. (Thinking about the person, what they do well, what they like to do and what their hopes and dreams are). • We will support and help them to look at ways to have more choice and control in their lives.

  4. Our Aim • We want to look at things like: • Having choices about your care and support. • Having things to do in the day, evening and weekend. • Having a Job. • Being healthy. • Being safe. • Where you live. • Support for Family Carers

  5. Why Are We Doing This? There are 2 reasons why we are doing this: • We need to deliver better outcomes for people with a learning disability and their carers. (The goals a person wants to reach to meet their needs). • We need to save £4 million over the next 3 years.   6. How we will do this? More Choice and Control • Self Directed Support (this is about you being in control of the support you need to live the life you choose) • Personal Budget (this is a sum of money to pay for services and support which meet your needs).

  7. How we will do this? A Fulfilled Life • Daytime and community activities

  8. How we will do this? A Place to Live • More supported housing options

  9. How we will do this? Good Health • Helping people to stay healthy

  10. How we will do this? Support for Family Carers • Carers assessments • Short breaks • Planning for the future.
