Hospital Indemnity Critical Illness Supplemental Life Insurance Vision • New Insurer: MetLife • Associate must enroll to have dependent coverage • Basic plan participants must enroll to have coverage • New Insurer: MetLife • Associate must enroll to have dependent coverage • Guaranteed Issue: No EOI • Currently covered: 5x annual earnings or $500,000 • New enrollees:1x annual earnings • Higher Allowances! • Frames: $230 (was $200) • Contacts: $180 (was $120)

HEALTH SAVINGS ACCOUNT by Fidelity • Sysco contributes money • $250 associate only coverage • $500 all other coverage tiers • Another pre-tax savings vehicle • Contributions are not taxed when used for health expenses • Unused HSA dollars roll forward • Not a “use it or lose it” account • Keep the money, even if you change plans or leave the company • Invest savings to earn even more • Note: Only available for HSA medical plan participants

SUPPLEMENTAL LIFE AND AD&D by Prudential For You Supplemental Life Insurance Voluntary AD&D Insurance • You may elect up to 10x your previous year’s eligible earnings • $1,000,000 plan maximum • You may elect up to 8x your previous year’s eligible earnings • $1,000,000 plan maximum There is no proof of good health requirement (EOI) up to the lesser of 5x or $500,000 for you, if you are currently enrolled in supplemental life.
