  1. If all the criteria below are satisfied then a referral can be made to the Young Carers Project. How to refer a young carer to the Young Carers Project The young person is carrying out significant caring tasks and assuming a level of responsibility for another person, which would usually be taken by an adult? The young person is responsible for the care of a member of their immediate family who has a physical illness, disability, mental health problem or dependency on alcohol or drugs The young person is aged between 10-16 years old and is the MAIN CARER The young person is living in the borough of Harrow. A referral can be made by post/telephone/email/fax To the Young Carers Development Worker 25-27 Pinner Green, Pinner, Middlesex. What information to include in a referral to the young carers project: • Name • Address including postcode • Telephone Numbers (Home and mobile) • Email Address • Date of Birth • Gender • Hours of Caring per week (approx.) • Area of Care (Learning Disability, Physical Disability, Mental Health, Elderly, Terminal Illness) • G.P Information: Name of Surgery • Ethnicity: (There is no obligation to complete this area, for monitoring purposes only) • Cared for details: Name of cared for • Date of Birth • Relationship to carer • Name of surgery (if different from above for carer) • School/Employment • Contact Name • Address including postcode • Telephone Number • Email Address • Who the Young person was referred by • Name • Contact Number • Agency/Organisation
