Background • Summer 2012 • Meeting with Director of Health Disparities, Utah Department of Health • Joint collaboration on community outreach and providing access to health care for underserved population • Latinos, Pacific Islanders and American Indians • Low socio-economic status • Loss of funding 2010-2011

  3. Opportunity • To serve and give back to a community that needs health care and education • Inter-Professional Education :for all of Health Sciences to work together as one in the service and education of the community • For all health professionals to learn about cultural competency as we experience treating patients from a diverse background.

  4. Purpose • To provide health care to the midvale community, with strong need for family practitioners, internists, pediatricians and mental health professionals • Long term goal : Midvale Clinic will serve as the flagship for the University of Utah Health Sciences in community service, inter-professional education and cultural competency

  12. Midvale CBC Community Clinic • Report: June 22 to Dec 5, 2013 • 3 trial clinics and 6 regular clinics. TOTAL: 9 Clinics • DATE NUMBER OF PATIENTS • June 22, 2013 4 • June 29, 2013 8 • September 21, 2013 8 • October 14, 2013 8 • October 21, 2013 10 • November 12, 2013 16 • November 19, 2013 10 • November 26, 2013 9 • December 5, 2013 5 • We have had a total of 78 patients attended by two providers. • According with the providers, patients need specialist in the following areas: • Specialty Patients referred • Dermatology 3 • Gastroenterology 1 • Physical Therapy 5 • Nutrition 2 • Gynecology 1 • Laboratories have been ordered from Oct 21, for a total of 11 orders.

  13. Patient Demographics White : 4 Hispanic : 72 Hispanic/white : 8 Asian : 4 Native : 1 Total = 88 According with the Providers, 14 patients waiting for specialists in the ff areas: Dermatology : 3 Gastroentorology : 1 Nutrition : 2 Gynecology : 2 Physical Therapy : 6
