We will start Reading Program at 3:00 and lunch will start at 3:30.

MIDWAY FOOTBALL Player Commitment Contract PLAYER:__________________________________________________ YEAR:____________________ Commitment Oath: I understand that participating on the Midway Football Team is a privilege, not a right, and I must earn this privilege through dedication to the TEAM’S SUCCESS and to my FELLOW TEAMMATES. I will make football a priority by maintaining & constantly improving upon the following traits; Integrity- I will do the RIGHT thing whether someone is looking or not. I will not put myself in any environment or situation that could jeopardize my standing in the Midway Football Program. Pride- I understand that any and all actions on my part reflect on the Panther Football Team, therefore I will not do anything that would embarrass the team or my teammates no matter who I am with or where I may be. I will take great Pride in all that I do by adopting the attitude of getting better every day in every way. Responsibility- I will take responsibility for all my actions. I will not pass blame on others or make excuses for my behavior or results. Loyalty- I will be loyal to this team by never bad-mouthing coaching decisions or teammates’ performances to others. If I have a problem I will take it up with a coach or teammate on my own, man to man, face to face. Discipline- I will discipline myself so others do not have to. Healthy Lifestyle- I will practice healthy eating habits, drinking habits, and sleeping habits. I will eliminate behaviors that contribute negatively to my performance and overall health. Punctuality- I will be on time for all practices and meetings. I will not schedule appointments during practices unless it is an absolute emergency. Student Athlete- I will work hard paying attention to the details and study hard for all my classes. I will attend tutoring when necessary and take part in athletic tutoring when assigned. I will respect the school, the faculty, my coaches, and my teammates by… *Understanding that I am a student first and an athlete second. Taking care of my business in the school building is my top priority. *Showing respect to all adults, teachers, administrators, and staff members by my speech and my actions. *Following all MHS behavior guidelines including the MHS Dress Code. *Being a role model and leader in the building and anywhere in the Midway-Waco area. I am held to a higher standard as a member of the Midway Football Team and will act accordingly. *Not participating in any social behavior that is in violation of any state law or district policy. *Attending all classes and arriving on time ready to work.

MIDWAY FOOTBALL Player Commitment Contract As a member of the Midway Football Team, I am expected to adhere to the following policies: Absences All Sophomores, Juniors and Seniors must call in prior to 12 noon if they are to be absent. Freshmen must call in before 7:00 am. (254-761-5670 extension 1) Athletes will make up any conditioning that they miss due to an excused absence. Unexcused absences will result in Level I Discipline. Athletes will attend practices and games even if they are unable to participate. Injuries Any injury, no matter how minor, will be reported to Coach Shelton immediately following workout. Athletes will undergo and complete the necessary treatment BEFORE practice begins and will continue treatment following the end of the workout. Athletes are responsible for following the treatment schedule that is given to them. Injured athletes are still required to attend practices and games. Doctor’s or trainer’s clearance is needed before returning to play. Playing Time Athletes will understand that football is a TEAM sport and will accept their role as a team member. All players will be given playing time at the Freshman and JV levels as long as they have attended all practices and have had no discipline issues in the classroom or on the field. Playing time at the varsity level is NOT guaranteed. Quitting Athletes that quit may be allowed back out in the spring off season pending a grade and discipline review. Any athlete that quits a second time will not be allowed back on to the team. Player & Parental Concerns/Questions To ensure that MHS Athletic Policy and Procedure is carried out, all concerns, questions, or problems will be addressed in the following order: Athlete-Team Coach Athlete’s Parent-Team Coach-Head Coach Athlete’s Parent-Team Coach-Head Coach-Athletic Director Midway Panther Football Discipline Policies Level One: 100 yard bear crawl, 100 yard grass drill (Every 10 yards hit the ground) Unexcused Absence Minor Discipline Issue Locker Room Discipline 1st missed Athletic Tutorial Level Two: 200 yard bear crawl, 100 yard grass drill (Every 10 yards hit the ground) Behavior Unbecoming of an Athlete Skipping Athletic Period/Excessive Absences Any time spent in ISS 2nd missed Athletic Tutorial As a part of the Midway Panther Football Program, I understand that I must be committed to the TEAM and our relentless pursuit of EXCELLENCE! I have read and agree to this contract set forth by the Midway Panther Football Program. MIDWAY FOOTBALL For a Conference/Concerns • Contact your son’s Position Coach first. • If conflicts cannot be resolved with position coach please contact Coach Gambill at 761-5670 ext. 2618 • Please do not attempt to confront a coach after a game or practice. Emotions are running too high and will not result in a positive resolution. Coaches WILL NOT discuss playing time, play calling or other players.

MIDWAY FOOTBALL Parent Code of Conduct • Love and support your child and the team, win or lose • Be realistic about your child’s abilities • Be positive and encouraging at all times • ALL PRACTICES are open to the public. Feel free to come watch practice, however, do not enter confines of field, stay on sidelines for your safety. • Respect the coach’s decisions and encourage others to do the same • Realize that we are in this together!

  11. MIDWAY FOOTBALL Other Important Information Valuables: Players should leave all valuables at home. Every student/athlete is given a locker to keep their personal belongings-make sure you lock your things up. Policies: We follow all MISD School Policies governing student code of conduct. 1st STRIKE= SUSPENSION for 10% of schedule 2nd STRIKE= Removal from all extracurricular activities for 1 calendar year NO PASS/NO PLAY: Any student/athlete failing a class is ineligible for participation in UIL contest (but will be required to practice). MISD allows students to request a no pass/no play exemption 1 time per semester only for Pre AP/AP classes identified by MISD. 1. The students 6 weeks average must be between 65-69 in the Pre AP/AP class. 2. Student must have attended tutorials. 3. Student must fill out waiver form from office. Parent must sign it. 4. Principal must sign & approve. Public Record Policies: Please be advised that interaction via facebook, myspace, cell phone, emails and any other media is ALL PUBLIC RECORD and subject to MISD discipline. Transportation Policy: All JV/Varsity players must ride the bus to and from all contests. Family emergencies must be cleared through the coaching staff. Freshman may sign out with their coach if they are playing in the 1st game of the night.

  12. MIDWAY FOOTBALL Quarterback Club The Quarterback Club meets every Monday Night in the DLC at 6:30. This will be a positive experience-no negative comments will be allowed. Everyone is invited!!! Please come show your support for the Panther Football Program!!!

MIDWAY FOOTBALL EQUIPMENT INFORMATION • YOUR SON WILL BE ISSUED THE FOLLOWING FOR $20: • Practice Shorts & Shirts • Practice Jersey & Pants • Girdles & Belts • All Hip-Thigh-Butt-Knee Pads • Game Jersey and Pants • Shoulder Pads • Helmet & Mouth Piece • Locker & Lock • Varsity Only-Game Shoes • This equipment is property of MISD and must be returned. Your son will be required to replace any lost or damaged equipment per MISD policy

  14. MIDWAY FOOTBALL EQUIPMENT INFORMATION CONTINUED All players need to purchase cleats. Varsity players will be provided with game shoes.
