  1. N JO 7210.633Operational Error Reporting, Investigation and Severity Policies Target Audience: Terminal Operational Personnel Effective June 25, 2007

Background • Terminal operational personnel were previously trained on N JO 7210.645, Operational Errors Severity and Proximity Events, which took effect 2/26/07 • N JO 7210.650, which took effect 4/30/07 canceled N JO 7210.645. • N JO 7210.663 is the latest change and will become effective on 6/25/07. It will cancel N JO 7210.650.

Background (cont) • N JO 7210.663 changes: • The major change for the control workforce is that Proximity Events (PEs) are no longer considered Operational Errors (OEs) • N JO 7210.645 changed Category D OEs to Proximity Events, but a Proximity event was still considered an error • Training documentation and reporting requirements for PEs will be different than those for OEs and ODs • The definition of an OE has changed

  4. Proximity Event (PE): Definition • A loss of separation minima where 90 percent or greater separation is retained in either the horizontal or vertical plane, and does not include any violation of wake turbulence separation minima. A Proximity Event is not an operational error.

  5. Operational Error (OE): New Definition • Less than 90 percent of the applicable separation minima results between two or more airborne aircraft… • N JO 7210.663 • Used to say “Less than the applicable separation minima…”

  6. PE: Employee Notification • Employees are not required to be verbally notified that they have been associated with a possible PE • Employees are required to be verbally notified that they have been associated with a possible OE/OD

  7. PE: Training Documentation • No requirement for a separate page in Section VI of the 3120-1 • The following training items must be entered in the 3120-1 without reference to the PE • Any associated remedial or skill enhancement training • Any associated recertification

  8. PE: Different Reporting Form • Possible OMIC/OSIC/CIC responsibility • Proximity Events will be reported on a different reporting form than the 7210-2 used for OE/ODs – The PE reporting form will be 7210-6 • Faxing Form 7210-6 to the ROC/WOC within 4 hours of the PE. Verification that the fax was received is the only required notification for PEs.
