    1. Kelly Woodward, Lt Col, USAF, MC Population Health Integration Team POPULATION HEALTH IMPROVEMENT PLAN AND GUIDE

    2. Overview Optimization and the PHI Plan and Guide Framework for population health improvement PHI Plan and Guide Draft and Revision MHS population health support Resources for population health

    3. Optimization and the PHI Plan and Guide MHS Optimization Plan charters development of PHI Plan Imperative to shift from intervention to prevention

    5. Framework for Population Health Improvement Systematic approach to community health Combination of public health and health plan principles Evidence-based clinical and business practices Measurable improvements in health

    6. Systematic Approach Align with MHS Mission, Vision, and Goals Information-driven actions Health information management strategy Knowledge transfer Build population health capacity Population health support structure/processes

    7. Public Health and Health Plan Principles Assess health status and risk factors of defined populations--applied epidemiology Prioritize and target clinical and community health services--1o, 2o, and 3o prevention Forecast and manage demand Manage capacity

    8. Evidence-based Practice Effective health services--doing the right thing Clinical preventive services/community preventive services Clinical guidelines Condition/disease management Efficient health services--doing things right Case management Business case analysis

    9. Measurable Improvements in Health Actionable local measures that correlate with improved health outcomes Identify risk factors and recommended preventive services to level of PCM population Structure and process measures Near-term measures to support management Standardized and scalable health measures that support enterprise metrics

    10. PHI Plan Draft and Revision

    11. Current Draft On the web http://www.tricare.osd.mil/mhsoptplan/components/population_health_improvement.html Guide for MTF key process elements Enrollment processing (defining populations) Demand forecasting Capacity, demand, and condition management Community outreach Outcome analysis Introduces MHS Support Center Describes tools for population health

    12. The Department of Defense (DoD) PHI Plan provides guidance in support of a uniform health care system based on systematic business and clinical decision processes. This plan is a framework based on four main assumptions: The delivery of user-friendly, quality patient care is critical for the MHS success. Assimilation of best available evidence, judgment and experience are necessary to stay competitive. The best allocation of scarce resources will be achieved through the systematic application of continuously improving standards of care, and is paramount for survival. Ideal delivery of health care within the MHS depends on leveraging the capacity of contract partners with the direct care system. This Figure shows the overall process involved with PHI. It demonstrates the relationship of the various process elements associated with PHI to the completion of the MHS mission. There are two factors to the equation: the demand on the system and the capacity of the system to complete the mission. Basic laws of economics state that there is a gap that will exist between these two factors. Doctrine or management paradigms exist to minimize this gap. 13. PHI Plan Revision Updated PHI Plan due in Spring 01 Intended to be a living document Primer on PHI; combines Community approach and Traditional health care services Retains guide to MTF key process elements Emphasis on linking users to tools/resources (e.g., population health support offices)

    14. MHS Population Health Support Developing office at TMA Coordinating population health initiatives Knowledge management and transfer Facilitate health information analysis/dissemination Work with Region population health offices? Services continue to build population health support capacity
