Requirements for contractor/company: 1. Must have a $25,000 surety bond or letter of credit 2. A licensed individual must have passed the install/repair/upgrade/closure exam. Requirements for individual testers: 1. Must have one year of experience within the three previous years of making application to install/repair/upgrade/close; 2. Must provide three owner references 3. Must pass an exam. Application fee: $300.00 for company/contractor and $150.00 for individuals; Renewal fee: $300.00 for company/contractor and $150.00 for individuals. Renewals occur every two years and require sixteen hours of continuing education from an authorized training provider along with the completion of three jobs during the two-year renewal period.
