About us The Operation Sacred Trust ("OST") Program is charged with bringing unique approaches, experiences and expertise to serve homeless and low-income Veterans in Miami-Dade and Broward counties, located in the heart of South Florida. Purpose Built Families Foundation’s Operation Sacred Trust (OST) collaboration is a nationally accredited, public-private partnership for homelessness prevention and rapid rehousing. Presentations are communication tools that can be used as demonstrations, lectures, speeches, reports, and more.

Emergency Housing Solutions • The Emergency Housing Solutions program utilizes federal funds to support communities in providing street outreach, emergency shelter, rental assistance, and related services • This program provides resources for adults and families with children experiencing or at-risk of homelessness

  6. Permanent Housing Solutions • Permanent supportive housing is a proven solution to homelessness for the most vulnerable chronically homeless people • It pairs housing with case management and supportive services

  7. Rental Eviction Assistance • The first step renters should take if they are concerned about paying their rent or facing eviction is to contact their on-site manager or lease agent by phone or email right away to let them know • If you need rental assistance or a payment plan for your next monthly rental payment

  8. Care Management • Care management is a set of activities intended to improve patient care and reduce the need for medical services • By enhancing coordination of care, eliminate duplication, and helping patients and caregivers more effectively manage health conditions

  9. Relationship and Social Strengthening • Here are some ways to start: • Focus on your most meaningful relationships • Choose activities to do together that are most likely to bring joy to you and the people you care about • Delegate or discard tasks that eat into your time, or do them together with family or friends

  10. Employment and Entrepreneurship Services • It means turning to a specialty-services entrepreneur who knows how to get the job done right • Employment is a relationship between two parties, usually based on a contract – one being an employer and the other being an employee

  11. Peer Mentorship • Peer mentoring is a relationship between people who are at the same career stage or age, in which one person has more experience than the other in a particular domain and can provide support as well as knowledge and skills transfer • Peer mentoring may be a one-on-one relationship or experienced in a group

  12. Education and Literacy Programs • Literacy program supports the development of reading skills in young children • Community Action – Mobilizing parents and communities to support children as they learn to read through fun out-of-school literacy activities and through the creation of locally relevant reading materials

  13. Legal Services • Legal advice include drafting legal documents or contracts that affect the rights of a person, representing someone in a court of law or in a legal matter, negotiating the rights of a client, and advising someone about specific legal matters

  14. Benefit Assistance • Major Veteran Benefit Programs. The VA has a number of programs providing financial, medical and other assistance to veterans

  15. Healthcare Support • All services performed, provided, or arranged by the services to promote, improve, conserve, or restore the mental or physical well-being of person
