    2. 2 What are Long-Term Support Systems? Individuals, public and private agencies that deliver, oversee, screen for, or pay for services for seniors, children and adults with disabilities who seek some type of assistance with activities of their daily lives.

Families Friends and neighbors The Medicaid program Services and institutions for individuals with mental health issues Services and institutions for individuals with intellectual disabilities (formerly mental retardation) Services and institutions for seniors Social services Health services Hospitals Nursing facilities Assisted living facilities Continuing Care Retirement Communities Adult Foster Care

Long term support systems are fragmented Planning, management and administration of related activities are not coordinated as well as they could be Housing and long term support agencies plan and deliver services in different systems Modern methods for gathering data are limited Some people have no services at all, and others have less than they may need The number of Virginians over 60 will double by 2030

    5. 5 Why Are We Ready Now? Three key initiatives: Systems Transformation Grant Money Follows the Person Rebalancing Demonstration State Long-Term Support Profile Tool Grant (to measure improvements in quality of long term supports over time)

    6. 6 Why Should I Care? You may know someone using long term supports right now who is not happy with their life, their choices, or their living situation. You may work in the long term support system and experience frustration in assisting people to get what they need. You, your family members and your friends may need long term supports some time in the future.

    7. 7 OK, So What Will This Transformation Mean for Me? Effective this summer: Instant access to comprehensive information about supports More choices for community living New Medicaid “waiver” services to support you in your community if you qualify

    9. 9 Information Virginia Easy Access: One website you can visit to get information about any supports you might need including: Customized searches through VirginiaNavigator for supports near you E-mail support by 2-1-1 VIRGINIA

    10. 10 Information Rather get information by phone? Call 2-1-1 Virginia: A call specialist will assist you by telephone 24 hours a day, 7 days a week, 365 days a year

    11. 11 Faster Access to Supports Through Development of… An on-line Medicaid application Streamlined processing of applications for certain Medicaid “waivers” An on-line form that you can fill out to tell you if you might be eligible for certain supports

    12. 12 No More “Agency Hopping” Local agencies are currently joining together in 11 “No Wrong Door” implementation sites to improve your experience in obtaining and providing long term supports Goal is a network of No Wrong Door resource providers statewide by 2010

    13. 13 No More “Agency Hopping” The vision: A comprehensive network of aging and disability service providers that collaborate and plan together Electronic sharing of information with your permission to avoid your having to fill out multiple applications for supports at multiple agencies

    14. 14 Planning Based on Individual Needs and Preferences All systems will put individuals at the center of planning for long term supports. Individuals will control more of the services and supports they use. Family, friends and neighbors can play a more active role supporting the people they care about.

    15. 15 More Choices for Community Living If you live in a nursing home, Intermediate Care Facility for Individuals with Mental Retardation (ICF/MR) or long stay hospital, you will have more options for moving to the community if that is your choice. If you currently use waiver services in the community, you will be able to access new waiver services that you need.

    16. 16 New Medicaid Waiver Services Under “Money Follows the Person” Transition Services – a one-time, life-time maximum $5,000 benefit for individuals transitioning from institutions to the community for up-front household expenses Transition Coordination - responsible for supporting the individual with the activities associated with transitioning Local government agencies, private non-profit 501(c)(3)s and fiscal services agencies Same rate as MR case management Consumer-Directed Supported Employment - consumer-directed option for individual supported employment services

    17. 17 Services Added to Medicaid Waivers Under “Money Follows the Person” Elderly and Disabled with Consumer Direction (EDCD) Environmental Modifications Assistive Technology Transition Coordination Transition Services Technology Assisted (TECH) Personal Emergency Response System (PERS) Transition Services HIV/AIDS (AIDS) Environmental Modifications Personal Emergency Response System (PERS) Assistive Technology Transition Services

    18. 18 Services Added to Medicaid Waivers Under “Money Follows the Person” Individual and Family Developmental Disabilities Services (DD) Consumer Directed Supported Employment Transition Services Mental Retardation (MR) Consumer Directed Supported Employment Transition Services Day Support (DS) Consumer Directed Supported Employment

    19. 19 Additional “Money Follows the Person” Services Supplemental home modifications Rent to cover time between signing a lease and moving in after home modifications are completed 24-hours-per-day, 365-days-per-year emergency back up service provided by 2-1-1 VIRGINIA for the first 12 months after someone moves back into the community

    20. 20 Closing the Housing Gap Housing, transportation and long term support stakeholders formed a Task Force Developed an Annual Housing and Transportation Action Plan to increase options for seniors and people with disabilities in September

    21. 21 Closing the Housing Gap Annual Housing and Transportation Action Plan addresses ways to: Create affordable housing Make accessible transportation available Connect housing and transportation Develop lasting partnerships among housing stakeholders and human services system stakeholders

    22. 22 Closing the Housing Gap 16 Centers for Independent Living are working with local housing and human service agencies to: Address the housing needs of individuals with disabilities and seniors Assure that these needs are incorporated into housing planning Track related policy changes

    23. 23 Keys to Success Working hand in hand—state and local agencies, private and public providers, advocates, people who currently use long term supports, and families A common vision and goals A passionate belief that we can do better

    24. 24 Some Questions You Might Have Do I really have choices about my supports? Yes, you do. Do I have a right to choose whether I live in an institution or in the community? Yes, you do. Do providers need to honor my choices? Yes, they do.

    25. 25 More Questions You Might Have Will transformation create more community living options, while keeping institutional living options? Yes, it will. Will people have more opportunities to use their job skills in institutions and in community systems as a result of transformation? Yes, they will. Will there be safeguards for people who want to live in the community? Yes, there will.
