  1. Skyline Middle School Technology EducationStudent Travel Agreement Form “While traveling and attending conferences and field trips with the Skyline Middle School Technology Education program, I understand that I will be required to abide by the following:” • All Skyline Middle School rules and guidelines are to followed AT ALL TIMES. SPECIAL NOTE: If a student chooses to misbehave, or makes a serious lapse in judgment, and violates the guidelines as they have been established, he/she may be sent home early, at which time parent(s) shall be contacted. In extreme situations, parents will be asked to come to the conference site to pick-up their child. In these instances, the student will forfeit all fees and contests, and will not participate in any future trips or events. • All TSA Event guidelines, practices, and procedures are to be followed AT ALL TIMES. • All Technology Education department rules, guidelines, and safety policies and procedures are to be followed AT ALL TIMES. Safety requires doing as well as knowing. Leave areas cleaner than you found them, and so on. • Absolutely No loitering after 11:30pm (Our curfew time). This means NOT being out of your assigned room for any reason unless designated an emergency situation or other circumstance approved by your advisor. In an emergency, advisors should be notified as soon as possible. REMEMBER: You MUST get a good nights sleep to compete well, so get some rest! • Always travel in pairs while attending events and activities – It is always safer to explore in pairs than singly. We will discuss and will implement a “buddy” system. You are expected to follow this process when possible and practical. We understand that there will be times when this is not possible, such as when getting to an individual contest or event, however, in these cases, make sure others know where you are, when you are leaving, and when you will return. DO NOT simply wander away from the group without a buddy for no acceptable or applicable reason, such as to get a snack, or to “look around”. Personal time is important, but, take time for this when you are safe at home and not on a school related trip. • Students may leave the hotel ONLY when accompanied by an advisor, chaperone, or a parent. In any case, the advisor is to be notified as to when, where, and who is leaving the hotel, and why. • Each room will have a group / team leader responsible for getting everyone assigned to that room ready for TSA events at the right time and in proper attire. Team leaders will be assigned by the TSA Advisor(s). • REMEMBER… Listen to your conscience… if you are getting ready to do something questionable, simply don’t do it. “Err on the side of caution.” If unsure, then ask an advisor, or chaperone. If you feel that we may say no, then ask FIRST, and be willing to accept the answer given. Sometimes a reasonable alternative might be available. THINK SAFETY & RESPONSIBILITY FIRST! “I understand the above, and will do my best to abide by these guidelines to the best of my ability.” Print Your Name:________________________________ Signed:_______________________ Date:___/___/___
