  8. Who is Social prescribing for? • Mental wellbeing which is affected by social circumstances • Vulnerable and at risk groups • Mild to moderate depression and anxiety • Chronic physical and enduring mental health conditions • Frequent attenders in Primary care

EVERYBODY …ALMOST • Patients experiencing acute crisis, acute psychosis or severe depression • Patients who are actively suicidal • Patients with substance misuse as the primary problem • Patients with a history of violence where there are ongoing concerns for the safety of the lone working link worker

Health services should be part of the whole health agenda, rather than be separate from it… A whole area based approach which I do feel is essential because we can’t cure people because a lot of their ills aren’t illnesses – they are dis-ease rather than disease.” -GP
