  1. Affidavit of Service Pennsylvania’s Protection From Abuse Database

  2. You can create an Affidavit of Service by checking the box next to Other Documents on the Case History screen. Click on the link for Affidavit of Service. You can also print out a blank affidavit form to complete by hand rather than using the online form. Affidavit of Service

  3. The caption should carry over any information from the filed orders. Affidavit of Service

  4. Enter the name of the person who attempted to serve or served the Defendant. You can select either served or attempted to serve from the drop down list. Affidavit of Service

  5. There are three options regarding service. You can select that service was made, by whom, when and where. You can select the option that service was not made but was attempted and enter any details in the box provided. You can select that service was made via mail and enter the details regarding this service. Affidavit of Service

  6. You can enter any notes regarding attempted service or service in the Additional Notes box. Enter the title of the person who attempted to serve or served the Defendant. Affidavit of Service

  7. The completed affidavit opens in a new window. Make sure you don’t have a pop-up blocker! The Case History button is your save button. Always use the navigation buttons provided in the database rather than on your browser tool bar. Affidavit of Service

  8. Use the File, Print options to print from this window. Affidavit of Service

  9. The affidavit will remain “unfiled” until it is filed by the county filer. You can make changes to it until it is filed. Affidavit of Service
