DD FORM 1348-1AISSUE RELEASE/RECEIPT DOCUMENT • The DD Form 1348-1A is the primary document that establishes accountability for property. • It will be provided to you by the DRMOs when you pick up property, or attached to the shipping box if the property is shipped to you. • You must maintain this document throughout the accountable lifecycle of the property.

FAX AND RETAIN • Fax or E-mail a completed copy of the DD Form 1348-1A to our office within 30 days of receipt of property. This will allow us to complete the property receipt process. • RETAIN and MAINTAIN a copy of your DD Form 1348-1A in accordance with the State Plan of Operation Records Management Policy.
REQUEST FOR DD FORM 1348-1A • If you did not receive a copy of the DD Form 1348-1A, fill out the request for DD Form 1348-1A form and Fax/e-mail to the DRMO from where the property came. • If the DRMO cannot furnish you a copy within a reasonable time frame (14 days), fax to our office a receipt of the fax/e-mail that was sent to the DRMO, and we will obtain a copy for you if possible. • Why not just send the request directly to our office? Doing so will only cause the DRMOs who are not providing the DD Form 1348-1A to continue to do so and the system will never improve. • A copy of this request format is available on our website under agency resources tab.
