In order to provide safe care for Cystic Fibrosis patients and to prevent cross infection with clonal/epidemic strains of pseudomonas aeruginosa , a policy of segregation was introduced in the University Medical Centre Utrecht Department of Pulmonology Nursing aspects of segregation policy • Aim • Exploring the impact of segregation on nursing care and to share this • experience with other professionals in the field. • Methods • Describing nursing aspects that require extra attention . • Results • Segregation policy means the following measures. • Admission of Cystic Fibrosis patients is preferably in a single room. • The door of the patient room is closed. • Every one who is entering or leaving the patient’s room must use hand disinfectant. • The patient is not allowed to leave the room except if it is necessary for treatment or medical examination during daytime. • If the patient leaves his/her room he/she has to wear a nose/mouth mask in order to prevent direct and airborne transmission of pseudomonas. • The instituted segregation policy has several implications. • Because of solitary nursing communication can decrease . • This may have psychosocial consequencessuch as social isolation. • During daytime patients are confined to their room, apart from physiotherapy sessions. • This has also physical consequences. • We have to put fitness equipment in the patientroom. It requires a lot of self discipline to exercise especially when the patient is not feeling very well. • Sometimes there are no single rooms vacant and we have to admit the cystic fibrosis patient in a 2 or 4 bedded room. It means fellow patients are confronted with the segregation measures. We had to develop a way of informing every patient and their visitors of the measures.. • The professionals were facing a whole new approach of the Cystic Fibrosis patient.
