  4. Staff are responsible for all thepinkareas on the application EXCEPT SECTION 1 If student is with you at the counter, we do not need I.D. at this point; inform student they will need it for everything else. Leave Blank

  5. Section 2: Course of Study (Enter Program Code) Enter Program Code From Registration Manuel Leave Blank.

  6. Citizenship & Residency Coding • If Answer is Yesto Both US Citizen & WA Resident, Enter Y 1 01 • If Answer is Yes to Only US Citizen, but NO to WA Resident, Enter Y 2 02 • ALL OTHERS, Enter ZZ 3 33

  7. Section 3: WA Residency Info. If “No” to 12 Months in WA, but, more than 12 months in box, code 3 33 & Leave Citizenship Y Whether “Yes” or “No” to 12 Months in WA, Check Question #2. If “Yes” to #2, use status of parents. Look for Yes or No

Section 4: Race & Citizenship Info. • If Answer is Yes to Both US Citizen & WA Resident, Enter Y 1 01 • If Answer is Yes to Only US Citizen, but NO to WA Resident, Enter Y 2 02 • ALL OTHERS, Enter ZZ 3 33 If International Student, Adm. Number is 44444 If Citizenship is Yes, Enter Y, If it is No, Enter ZZ & 3 33

The Only Exceptions for “NO” to U.S. Citizen (Always get Copies) • If student is not U.S. Citizen, but are with you at counter with proper documents, you may update in HP ONLY. • In addition to answering “Yes” to 12 months in WA, student must have 1 of following: • Permanent Resident (Green) card/1 year issue date. • If card is not 1 year old, student may show “Green” card application showing “Application Received Date” of 1 year • Code IM 1 01 • I-94 Card for Refugee/Asylum: Code RF 1 01 (If “No” to 12 Months in WA, code RF 3 33 in SM 2001, and then RF 2 23 in SM 700A only for only 4 consecutive quarters)

  12. Section 7: Other Admissions Info. Always 77777 Except International is 44444 Always 1 Always NC W, or X when with Student at Counter Stu. Type Depends on Program; If None, Enter 2 Leave Blank; Updates Automatically Enter Your Initials; Click Save & Approve

  13. Student May Already be in the System! Go Back to Application; Put on Hold; Do not Process Only if student is at front counter with photo I.D., get full-time staff to research and absolutely verify it is the same record to enter S.I.D. in app. & “Save and Approve.”

  14. Paper Application in SM2001 YRQ Plan start date/ Personal/Citizen/Res. Info. If student is with you at the counter, we do not need I.D. at this point; inform student they will need it for everything else. If International Student, Forward Student or App. to ISP

  17. If Student Can Register Immediately:Go to SM 5027 Enter Birthday for P.I.N. mmddyy

Also at the Front Counter, Give S.I.D. # & Admissions Booklet NORTH SEATTLE CC SID # _____985-00-0000_______________ YOUR PIN NUMBER: TO KEEP YOUR RECORDS SECURE, PLEASE CHANGE YOURTEMPORARY PIN # AS SOON AS POSSIBLE Note: When you enter C or X for Stu Enr Stat at the counter , you give these doc., when you enter 1 or W for Stu Enr Stat, this is how we know to send these doc.

  19. ESL Always complete appl. for student according to Photo I.D., if they have it, to ensure accurate name; verify current address, apt. #phone #. Always ask Visa Question; if No, get initials; if Yes, send student to ISP If not ISP; process in SM2001; after assessment, schedule for test, or place as level 1 (ES011); if returning from within the past year, copy transcript (SM6015) and send to Advising. If student is with you at the counter, we do not absolutely need I.D. at this point; but inform student they will need it for everything else.

  20. For ESL, process the following open fields only; none that are crossed out If Testing, type E then test date (E1109) If not ready for test, set for level 1, type ES011 Stu Enr Stat for ESL students is E Reminder: if returning from 1 year or less, print transcript, (sm6015) send to Advising; new students, schedule as appropriate for test or level 1 orientation.
