  1. Welcome to the Reviewing Client/ Chart Copies of Immunization Records lesson for the North Carolina Immunization Registry. This lesson is only intended for all User Roles

  2. From the home page you will need to click on Manage Clientand then search for your client’s record. From the Demographic Screen you can view a client’s immunization report 2 different ways. (1) you can click on the Reports button from this screen or (2) click on History/ Recommend and then go to the reports from there.

  3. From here you can choose to print out a Chart Copy by clicking on Immunization Record-Chart Copy • You can print out a Patient Copy by clicking on the Site drop down arrow and choosing your site and then click on Immunization Record-Patient Copy. • You must have Adobe Acrobat Reader in order to view these reports. The Vaccine Administration link is the vaccine administration form that you have probably been using in your office for a while. You should use this for a contingency plan in case of not connecting to the NCIR.
