NEW Early Help Assessment FormStep by Step Guidance Notes April 2016 These notes are a step by step guide to completing the Early Help Assessment recording Form following a conversation with a child and/or family, with examples of good practice. Download and save a copy of the NEW Early Help Assessment form – Signs of Strength before completing Do have a copy of the Early Help Assessment with you when you read these guidance notes. These notes covers all sections of the Early Help Assessment up to closure. There will be some quotes from actual Early Help assessments which, although in quotation marks, have been paraphrased to preserve anonymity. It is also included in this notes the contact information for the Early Help Hubs and a handy checklist. 1. Obtain parent, carer, young person, child, or family agreement (page 1) Please remember to include date It is ok to type the agreement However, we would expect a note to say where the signed agreement is stored, e.g. “Anthony Person – signed agreement in locked filing cabinet in secretary’s office” 2. Practitioner agreement (page 1) Please remember to include date, agency and email address It is ok to type the agreement. However, we would expect a note to say where the signed agreement is stored, e.g. “Anthony Person – signed agreement in locked filing cabinet in secretary’s office” TOP TIP: Agency is the name of the organisation or team that you work for e.g. if you are working in a school, write the name of the school or if you are working in the health sector, the name of the team that you are working with such Health Visitor, Midwife, etc. Always include an email address so an EHA URN can be returned to you once registered.

Record basic details (page 1) Child or Young Person Please include child’s full name, date of birth and full address including postcode. This helps everyone quickly identify information about a child 4. Record basic details (page 2) Parent/Carer/Key Adult Please complete as fully as possible We all want our records to be as accurate and up to date as possible so making sure we have the full address including post code and everyone’s data of birth is helpful in this section. 3. Record basic details (page 2) Other people at this address Please do include all other people living at the address. What works well in Early Help is working with the whole family so making sure we have identified everyone at the address is very helpful. Please include any adult children who may still be living at home, and any younger children not at school. TOP TIP - Carers Information If any of the adults or children are carers please include details. Some carers may not identify themselves as carers so do include in your conversations any health conditions adults and/or child(ren) in the family may have which require care. 4. Record Details of Other Parent/Carer (page 2) The relationship with an absent parent can be very significant for families we work with, so please do include this information if one of the parents does not live in the household.

Record services/professionals currently working with the family (page 2) Please complete as fully as possible Completing a list of all agencies working with every member of a household helps the family have a complete picture of the issues they are working through It also means we can avoid making duplicate referrals to agencies that may be already working with the family. TOP TIP - Need more space ? There is space to add more family members and/or services/professionals working with the family on page 4 6. Start the conversation - Whole Family Assessment (page 3) The conversation should always start with a focus on the families/individuals strengths. Complete this section AFTER you have held the strengths based conversation with the family. Do not use the form to drive the conversation - families can feel intimidated if they feel they are being formally "assessed“ Hold the conversation using the tools and resources from the Early Help Practitioner Zone web-pages and your local Early Help Hub. This is an executive summary of the key issues discussed during the conversation, so please do not feel you have to report the conversation verbatim (word for word). The plan should be developed WITH the individual and family, and they should have a say on who they want to involve in the process. The plan should result in actions that are about their life, not just referrals to services, and reflect what is possible, not just what is available. Please do complete the scoring sections this helps the family to prioritise areas they need to get going well again, and will help them to measure progress. TOP TIPS - Style Capture the voice of the adult/child - use first person [the child] drew a vision of a stable home using the houses tool Be non-judgemental “I’m tired all the time. I play a lot of computer games which is affecting my sleep” “Be factual – avoid hearsay “I often get into trouble with my teachers” Be brief – paraphrase – avoid word for word reporting “[the parent – name them] spoke about the families’ involvement with children’s services four years ago”

Actions and Who: (page 3) Please do make use of the tools and resources from the Early Help Practitioner Zone web-pages and your local Early Help Hub. TOP TIPS - Style State one action at a time - include who the action relates to (named individual or whole family) Build on Strengths - include actions where things are working well “home life scored highly with all the family, we agreed a film night every Friday to spend quality time together” Agree actions which are reasonable Agree actions which are achievable Be specific Use positive language Agree who is going to carry out the action Agree By When date (avoid ASAP - use a specific date) 8. Remember to set a review date (page 3) Set a review date to review the plan and actions or if you have reviewed the Early Help Assessment and all needs are met, a closure date.

Send the Early Help Assessment to be Registered We would prefer to receive these by email, you will get a unique reference number (URN) for your early help assessment for future reference.   6. NEW Early Help Assessment FormStep by Step Guidance Notes April 2016 12. When to use the TAF Planning Form? Use the TAF Planning Form if you are working with families with more complex needs (level 3 and 4), or when you are holding a Team Around the Family (TAF) meeting. This form will assist you to formulate an action focused and co-ordinated plan of support with the family. A review meeting date should also be set to come back as a group to review the actions and also to identify if there’s any new or emerging needs to be added to the plan. Please remember to include a date and URN (if known)so everyone know when this meeting took place and who it is for. If you do not have an URN then write the name of the subject child. TOP TIPS: Name of person – the name of the family member that this goal/action relates to Goal - What is the goal?What would positive change look like?If you wanted to make changes,and knew you wouldn’t fail, what would you start doing? Tasks/Actions - What are the specific steps to change? What needs to be done next? Person responsible - Which family members or professionals will be working on making things happen? Timescale/Review - Has the positive change been met?Next Steps - Does something else need to happen?What’s important for you and your family • 13. When to organise a TAF meeting? • When there is more than 2 agencies involved, a TAF meeting should be organised for the following reasons: • To ensure shared understanding of families needs • To jointly plan and agree on actions and outcomes • To ensure co-ordinated plan of support and to avoid any duplication of work • To support each other to overcome barriers • A person should be identified at the TAF meeting to co-ordinate the plan and to liaise with the family. Everyone at the meeting should get a copy of this plan to ensure everyone understand who is doing what and by when.

Next steps Begin the support and intervention Everyone should be responsible for undertaking their own agreed/planned actions and report back to the review meeting. The Lead Practitioner will liaise with family to ensure they are supported to undertake the actions they have agreed to in the plan. Review the plan and actions At the review meeting, all parties should report back on their planned actions and outcomes. The plan will be reviewed with score updated accordingly on each of the 5 aspects, and any new or emerging needs will also be recorded. Date of next meeting should be set unless all needs met and desired outcomes achieved, in this case, a decision should be made to close the EHA and a closure date should be set. • 16. Need support to move case forward? • In cases where the plan failed to achieve it’s desire outcomes due to the following reasons: • Family or practitioner disengagement • Family not undertaking the actions agreed on the plan (disguised compliance) • Case escalated due to family situation changed and targeted support required • You may contact the dedicated Early Help Hub for support and advice. Early Help Hub details can be found on page 5. 15. All needs met? Closing an EHA? Send closure date to Early Help Team to close an EHA When a decision is made to close the EHA, the person leading on the EHA or the Lead Practitioner should notify the Early Help Team of the closure by sending the closure date and the URN (if you do not remember the URN then send us the name of the subject child) to the following email addresses: If you have a gcsx account use this email eha@manchester.gcsx.gov.uk Otherwise, use this secure email eha@manchester.gov.uk
