  2. Make a simple list of High Utilizers • Open Quicklook Prevalence report • Type 1 in white rectangle under HU Visits header • Scroll far right to find column • Click on green funnel and select Greater Than

  3. Add notes to View Check box to add notes Detail to view Click HU Visits column header to sort then when done sorting, click again to sort high to low

  4. Save Custom template

  5. View Patient details • Click on patient then click Patient Detail Tab • Open High Utilizers Prevalence Report • Export HU Report to Excel • Must first have print/export privileges to see Excel Icon (request in Admin tab) • High Utilizers Summary not very different from High Utilizers

Suggestions for action • Case Management if appropriate • Connect with mental health provider if appropriate • Pain Management if appropriate • Schedule visits/calls every couple weeks so pt knows it is coming and gradually space them • Look for patients with frequent civilian ER or Urgent care or multiple MTF Clinics • Get PCM team to reach out and establish continuity
