  3. What should be reported in IRIS? Any occurrence, event, or happening that: • Is inconsistent with the routine care of a patient • Is inconsistent with normal hospital or clinic operations • Adversely affects patients, physicians, employees, contract personnel, or visitors • Has the potential to cause harm (including near misses) • For events with high severity or urgency, contact a CHI F Risk Manager to report the incident AND complete an IRIS report! Why complete an IRIS report? • To put Safety First • To promote quality of care • To improve performance • To identify trends and opportunities for growth • To report in a protected, trackable manner vs. an email report • Not to be punitive or take the place of direct communication Physician Orientation - IRIS

  5. When should an IRIS be completed? Complete an IRIS report • As soon as possible! • On the same shift in which the event or incident occurs and • No later than 24 hours following the event IRIS report completion is not documented in the medical record! Physician Orientation - IRIS

  6. How is an IRIS report submitted? From the CHI F home intranet – right side of the page: • Click on IRIS forFHS or for FMG(as appropriate) • Clickthe link to • Report a patient incident • Report a visitor/security incident • Report an employee injury or exposure • Follow the directions to enter event details Physician Orientation - IRIS
