  7. When Can I Change My Health Plan? • During open enrollment, Spring months enrollment, for an effective date of June 1st. • Qualifying events under Section 125 of the IRS Codes: • Change in Status such as: • Marital- marriage, divorce, separation, etc. • Dependents- birth, adoption, etc.

  18. Long Term Disability (LTD) LTD Plan Types: • 50% up to $1000/month • Offered to all eligible employee at No Charge • Waiting period: 180 days

FSA- Medical Reimbursement Account • Allows you to pay for certain medically necessary expenses with pre-tax dollars • Contribute up to $3500 per plan year ($50.00 minimum) • You may us the $$ even if not yet in the account • Examples: co-pays, eye glasses/contacts, orthodontia, X rays, etc. • Over the counter medications • Direct Deposit Reimbursements

  20. FSA-Dependant Care Account • Allows you to pay for dependent care with pre-tax dollars • Contribute up to $5000.00 per plan year per household ($50 minimum)
