  11. Individual Dental • Choose any dentist • Can be issued to individual if group falls below participation levels

  12. Vision Plans • Offered through VSP • Uses a network of VSP doctors – clients can elect to go out of network. • Guaranteed issue • No waiting periods

  13. Insured Vision Plan • May be combined with dental into one convenient package. • May be sold as stand alone with a minimum of 10 enrollees and minimum 75% participation of total eligible employees.

  14. Voluntary Vision Plan • Only three enrollees required • 12-month lens benefit • Two year rate guarantee
