Closure FTM: Celebrating Family Progress and Planning for Life Beyond DFCS

DESCRIPTION

The Closure Family Team Meeting (FTM) is an essential opportunity for families to reflect on their journey with DFCS and celebrate their achievements. This meeting focuses on identifying personal and family goals, additional resources, and aftercare services needed for a successful transition. It emphasizes proactive safety planning, relapse prevention, and maintaining self-sufficiency. Families will discuss lessons learned, new skills developed, and how they have grown stronger together. This collaborative process helps families prepare for a future beyond DFCS support.
