  4. Current Best Practices • Report of Accident (ROA) submitted to L&I in 2 business days • Activity Prescription Form (APF) when a worker has restrictions, cannot return to full-duty, or will be claiming time-loss • Provider contact with the employer • Barriers to recovery assessment

  5. Report Of Accident (ROA) • Purpose: • Allows a worker to access medical and lost work benefits • Allows the health care provider to be paid for their services • Focus on timeliness and completeness • ROA to L&I within 2 business days
