    2. Rural Counties are Structurally Deficient Most departments are one person deep For example, I am the director of 8 county departments. Lack of transportation Lack of Community-Based Organizations and Non-Governmental Organizations In some rural counties, the county is the only provider Lack of Professionals Many small counties have no full time psychiatrist, some only have tele-psychiatry

    3. Budgets and Cash Flow Community Mental Health funding is so bad, in many rural counties it only funds one staff member. There is currently not enough funding from the state or local governments to fund mandated services for the indigent, crisis, hospitalizations and IMDs (institutionalized chronically mentally ill). Over 30% of my total mental health budget is yet to be reimbursed by the state even though we are 60% into the fiscal year (thats $2.7 million as of January 30th). I have received no payments for Managed Care or Healthy Families.

    4. Despite All of this, There are Emerging Best Practices Benefit of being small is that we must collaborate and must be innovative. Because of MHSA, rural counties are delivering services in innovative ways: Drop-In Centers for youth, adults, and Veterans. Departments and Drop-In Centers are Consumer Driven: For example, 15% of my staff are consumers, both part time and full time with benefits. Consumers participate on interviewing panels for professional staff and have executive decision-making teams. To address transportation, lots of In-Home, Early Intervention strategies Mobile clinics Satellite Offices To address lack of funding, Supportive Housing for the chronically mentally ill to bring them home and support them in their communities (we can no longer afford institutionalization)
