Chart Key If Parent declines to participate Stop Or, Child is determined not eligible: Then document in central file and electronic record as a closed case; procedural safeguards and due process opportunities must be presented to family This is a moment to provide information on parent to parent support P2P Surrogate Parent This is a moment to identify whether there is a need for a surrogate parent.

Referral to Local Lead Agency Enter all referrals into EETRK Begin Development of Central File Lead Agency representative makes initial contact with the family and schedules intake appointment at the family’s convenience Acknowledgement of referral in writing or by phone to primary referral source Stop

Orientation and Intake P2P Meet Family; share information regarding Early On verbally and in writing; obtain informed written consent to proceed; start intake include releases of information Surrogate Parent Stop Request existing screening, medical and other relevant information to assist in eligibility determination Facilitate the identification of a primary medical care provider for child Assist family in completion of applications for public programs they may be eligible for P2P Develop composition of multidisciplinary team (two different disciplines) for eligibility determination based upon presenting child and family concerns. If eligibility cannot be determined with existing documentation, plan any necessary evaluations required for eligibility determination.

Eligibility Determination Assemble eligibility determination documentation, including for example comprehensive developmental evaluation data, medical information, parent report, observation and assessment summaries Multidisciplinary team (at least two disciplines) reviews documentation and jointly determines eligibility CHILD NOT ELIGIBLE: Link to larger Early Childhood System CHILD ELIGIBLE: Discuss and plan any necessary assessments for IFSP Development Stop

Needs/Service Assessment Carry out necessary child assessment activities needed for IFSP development Stop Service Coordinator and family develops the composition of the IFSP development team Facilitate the identification of the primary medical care provider for child • Service Coordinator meets with family to discuss IFSP process, document and assists the family in preparation including: • Family identification of priorities and resources; • Identification of daily routines and activities • Family input for IFSP components P2P Schedule and plan IFSP development meeting, notify all participants in writing Stop

IFSP Development Conduct IFSP development meetings, complete development of IFSP P2P Obtain informed, written parental consent to proceed and implement IFSP Stop Service Coordinator assists the family in IFSP implementation Notify referral source of results of the eligibility determination process and IFSP development with parental consent Implement IFSP; monitor, review and conduct six month review P2P Annual re-determination of eligibility, identification of Family Needs, develop new IFSP P2P
