MPO antibodies in Churg-Strauss Syndrome • 논문에 따르면, CSS 환자의 약 38%가 IFA 검사에서 양성.이 양성환자의 약 ¾이 MPO antibodies에 의함(30%) (Sinico RA et al, 2005, Arthritis Rheum 52:2926-35) • 자사 평가에 의하면 60명의 CSS 환자 혈청 중 23명에서 ANCA pattern이 나타남 (38.3%). 하지만 단 10명만이 IFA 검사결과 전형적인 pANCA pattern을 보임 (16.7%). 11명의 혈청에서 MPO antibodies가 검출됨 (18%). • EliAMPOS보다 민감도가 좋은 타사 제품은 없었기 때문에, 낮은 민감도는 EliA 때문이 아니라 sample panel 때문에발생. • 평가에 사용된 Churg-Strauss 검체에서 anti-MPO antibodies의 prevalence가 낮아 2번의 assay 결과를 가지고 계산 (with and without Churg-Strauss sera).

19. EliA MPOS – Comparison with IFA on granulocytes** An ANCA pattern was found in 63.2 % of ANCA-associated vasculitis (AAV) and in 29.3 % of controls. The classical MPO-associated pANCA pattern was found in 35.5 % of AAV and 2.7 % of controls. *x-ANCA: in most cases positive in ethanol-fixed granulocytes but negative in formalin-fixation. **Measurements are based on the MPOs serum panel (see slide 8) plus 30 additional sera from patients with Wegener’s granulomatosis in % Almost 30% of non-AAV are positive in IFA. pANCA is less sensitive and less specific than EliA MPOS. The clinical usefulness of IFA as screening test is questionable.

Disease control의¼ 이상에서 ANCA 양성 (granulocytes)의 결과를 보임. • pANCA는 EliA MPOS에 비해 민감도와 특이도가 모두 낮음. • 따라서, IFA의 임상적 유용성은 의문의 여지가 있음.

21. EliA GBM for detection of anti-GBM in ANCA-positive samples • ANCA 양성 환자는 GBM antibodies를 반드시 확인해야 한다! • ANCA 양성 검체의 5-14% 정도는 anti-GBM도 양성. Anti-GBM 음성인 vasculitis 환자보다 두 검사에서 모두 양성인 환자가 질환에 대한 예후가 더 심각.(Levy et al, 2004, see PoM 01/05) • 빠르게 악화하는 glomerulonephritis로 의심되는 환자 중 약 2%가 ANCA와 anti-GBM 모두 양성. 예후는 ANCA만 양성인 경우가 anti-GBM만 양성인 경우보다 좋다.(Jayne DRW et al, Kidney Int 1990; 37:965-70)
