  2. The Health Care Domains Model H2CM = “Hodges' Health Career Model” Originally created by Brian E Hodges (Ret.) MMU, Manchester A conceptual framework for educational portfolios Peter Jones Community Mental Health Nurse - Older Adults Nursing Home Liaison Specialist Beechurst Unit Chorley Lancashire UK 01257 245150

  3. Peter Jones - Background: Experience Older Adults Adult mental health Nursing Home Liaison Primary care projects Mentor / Sign-off mentor Care Programme Approach CMHP & GP Fund holding Mentor Public Health Analyst 3 yrs NHS Care Record Service RMN, RGN, CPN(Cert.) 4. Presentation: Aim: To introduce and highlight h2cm in reflection and student portfolios – structure & content Intro to h2cm: - Purposes – past and future - structure, content, knowledge & reflections? Examples Definitions: Textual, as 'white space', spatially Portfolios (some) matters arising • Help needed! • Strengths (3 slides) • Status • What next - research? Drupal CMS ... • Questions ...

  5. Purposes - past Hodges' model – created in the 1980s by Brian Hodges 4 educational objectives: Curriculum development. Holistic assessment and evaluation. To support reflective practice. To bridge the theory-practice gap. How? The model can be situated and person-centred.

  6. Structure: 1. Individual Questions: Who (subjects) 1 - 2. Actions (tasks) 3 - 4. H 3. M 4. 'H' = HUMANISTIC 'M' = MECHANISTIC 2. Group

  7. What knowledge do nurses use? Individual INTRA- INTERPERSONAL ? SCIENCES ? Humanistic Mechanistic POLITICAL SOCIOLOGY ? ? Group the SPIRITUAL domain comprises all four

  8. Example of the model – key concepts ...

  9. Another view: the 4-P's...

  10. Defining the model... h2cm is a conceptual framework It is not really a model, BUT there is a model and a host of theories implicit in the model's structure and content. H2cm is an open conceptual space. Open for the user to: • Contextualise • Situate • Personalise & Populate

  11. What this model of care is ALL about ...

  12. 1. Individual Personalised care is not a given. It takes work in theory and practice. This model reflects that. H 3. M4. 2. Group

  13. Purposes - future Multidisciplinary care Patient/carer engagement collaborative care Transitional & Integrative models Nursing model > Care model > Self-care model Disease, Self-care, Recovery Literacies – 'information' 21st century purposes ...

  14. … to build on h2cm's strengths The model is agnostic - • Skills • Disciplines • Speciality • Culture (age, gender, beliefs) • Media • Information systems (as yet) • Agencies (Public, Private, Voluntary)

  15. … strengths ... • Has growing relevance • Can add value to health and social care • Is free, open access, accessible • Simple – Complex; multicontextual • Users inc. Patient, Carer, Citizen, Student • Health AND Social care • Perspectives - problems, solutions, strengths, relapse prevention

  16. … strengths. • Professional values • Lifelong learning (beginner – expert [Ret.]) • Analysis – Synthesis : Reductivism – Holism • Local – Global: 'Glocal' • Socio–Technical (ICT systems) • Patient, Carer, Citizen, Student • Multiple hooks (ways to explain) • Reflection – models • 4P's • Stress – Vulnerability model* • Information processing models * Zubin and Spring (1977)

  17. H2cm: maps and bridges ... Cognitive periplus – a map A way to link what are usually disparate disciplines ====>> Nursing is also - Multidisciplinary? Education is - Interprofessional Learning is - Blended?

  18. Portfolios • Some aspects and content: • Models of reflection – as taught; • Elements of reflection; • Evidence to support learning; • Clinical practice reports; • Linking practice and knowledge; • Relevant assessment documents.

  19. Portfolios ... Reflective practitioners As students and qualified nurses Contents Process - guidelines Learning styles “finding a voice” Competencies An 'apolitical' space - speaking out Research(ers!)

  20. Portfolios: some issues? • How to assure assessment • Equal treatment in sign-off mentoring • Demonstrate the same! • Assessment of portfolios • Curriculum – change (speed, politics) • Appeals • Length of appeals/disciplinary processes • Reflective summaries (remote supervision) • Accord education ↔ service • High quality placements

  21. A bridge too far – help needed... Models of nursing (care) are “old-hat”! • Evidence based care • Lack of research • Website is dated: • static, no community, English only! • Lack of engagement - development • No feedback / no critique • Further study MSc. / PhD.

  22. Where is Hodges' model? Website 1998 – inc. Brian Hodges' notes Blog “Welcome to the QUAD” 2006 – ongoing (BUT comments disabled) Includes a bibliography, archive, tags Further papers: Forensic nursing Substance misuse Medical progress, information, 'progressive informatics'

  23. Old & New questions OLD: 'Holistic care' (What does this mean?) Patient outcomes Case formulation Theory of health communication Health Education Vs. Provision NEW: concepts demanding research? 'Holistic bandwidth' Informatics forms Literacies

  24. New site - New community! • Beyond the 1998 site and blog: • Community oriented • Database driven • Archive (Hodges' original notes) • Multi-lingual! • H2cm related Content types • H2cm resources (content types) • Taxonomies • Semantic web, RDF

  25. Drupal: Open source CMS • CONTENT TYPES: • Book (collaborative work) • Case study [care domains] • H2CM form, summary, matrix • Nursing problems, care plans, assessments • evaluations • h2cm taxonomy / vocabulary • Educational modules: Quiz, Bibliography • SCORM compliance? • Nursing Classification schemes • Student's terminologies (folksonomies)

  26. Research? • Conceptual Spaces (Gärdenfors) • Quality dimensions • 'Holistic bandwidth'? • Combining h2cm, informatics, and • programming • Domain specific language (internal) • Advice on formulating a question • Is there a Mentor in the house? • Dissertation and/or Project • Create new site then project • Specific educational purpose

  27. Presentation: Aim: To introduce and highlight h2cm in reflection and student portfolios – structure & content Intro to h2cm: - Purposes – past and future - structure, content, knowledge & reflections?
