Description of Activity Target Population • Children 0-5 who live at or near the federal poverty level and attend a child care site where they are income eligible for child care subsidy, and • North Carolina counties that have a rate of early childhood overweight/obesity among low income children that is greater than or equal to the state average of 29.7%. • Geographic Area • Counties with a high percentage of low income children ages 2-4 who are overweight or obese

Evaluation • SIF seeks to establish stronger evidence for promising interventions • Two evaluation studies to date: • Found a positive effect on the use of nutrition and physical activity best practices at participating child care centers • Most recent study successfully tracked Body Mass Index (BMI) of children over time • These results meet the SIF definition of Preliminary level of evidence

Award Terms Each Subgrantee must: • Conduct criminal record/background checks • Must complete National Sex Offender check on any employee paid with grant or matching funds before the start of work. • Must initiate a thorough state(s) background check and an FBI check if working with vulnerable populations. • This is crucial requirement of this grant. • Have an Accounting system in place that can properly manage tracking the Federal grant and dollar-for-dollar cash match requirement separately • Must be able to meet match requirement dollar-for-dollar by the end of each grant year and have 25% of the first year funds on-hand at the time of award. The match must be in new, unrestricted, non-federal cash; in-kind match is not allowable • Have the financial capacity to receive funds on a reimbursement basis

Award Terms Each Subgrantee must (cont): • Have the capacity to comply with OMB guidelines, SIF Classic Terms & Conditions, CNCS Terms and Conditions and NCPC Cost Principles. • Must be willing to be monitored by NCPC on all programmatic, operational and financial management aspects of this grant.

Selection Process • Evaluation Criteria (each application will be scored on the following criteria as outlined in the RFA): • Need –10 points • Capacity for Addressing the Needs and Implementing the Strategy – 25 pts • Evaluation Capacity – 20 points • Project Description and Implementation Plan – 25 points • Support Requested – 20 points • Disqualifying Factors • Submission of an incomplete application or an application received after the declared deadline of 5:00pm EST on December 9, 2016

  22. Questions During RFA Period READ THE RFA IN FULL ALONG WITH ALL OF THE APPENDICES An online list of Q and A’s will be posted and maintained during the RFA period. During the open competition, including before and after the bidders’ conference, all questions must be submitted in writing to Shape@smartstart.organd responses will be posted online. The FAQ list will be updated twice weekly on Mondays and Thursdays and a final version will be posted December 8th, 8:00 am.

Applications Must Include: • Completed Cover Sheet and Application Checklist (Appendix L) • Narrative Response to Scope of Work • Coaching Expectations MOU (Appendix F) • Support letter from applicant agency on agency letterhead: signed by your Board Chair and CEO/ED that demonstrates 1) 25% of the documented funds on hand, 2) a plan for meeting 100% of the first year cash match requirement by the third quarter, and 3) agency readiness to implement the requirements of the grant. 100% of the first year cash match must be met by March 14th, 2018 • Budget and Budget Narrative (Appendix K) • Liability Insurance Certification • NCPC Financial Management Survey (Appendix J) • Assurance and Certification Pages (Appendix M)
