1. Dropped Object : Pack-off Setting Tool Falls From Truck. Date: 1/25/10 Location: Hilseweck 2-6H (Drilling Location) Brief Account of Incident: Well head employee had concluded landing out casing. He loaded the tools he had used and went inside safety trailer to clean up. While he was inside, the rig crew loaded the pack off setting tool on his flat bed truck. Employee then returned to his truck and was driving across location when the pack off setting tool rolled off the back of truck. No one was in the vicinity of the truck when the tool rolled off. • Root Causes: • Driver did not perform 360° walk around prior to departure from location • Driver was unaware equipment had been loaded on truck • Lack of communication from crew to driver about equipment being loaded onto truck. Tool was placed on truck bed • Action Items: • Notify all drivers that 360° walk around shall be performed prior to leaving location. • Ensure proper communication is in place when materials are being handled. • Discuss importance of 360 when personnel go thru BP Orientation.
