  1. Parent or Educational Professional has concerns about a student. ASD Core Team Referral Process (Combined AIT & ADT Teams) Does Studenthave an IEP? Does the Student have Autism?* NO YES NO YES Initiate Discussion with your IEP Team (IEP Case manager, SLP, OT, Social Worker, School Psych, SIT Team). Initiate Discussion with your SIT Team (Principal, Regular Ed Teacher, SPED Teacher, School Psych). SIT Team 1) Determines need & recommends consultation. 2) Completes ASD Core Team Referral Form. 3) Obtains Parental signature on form (required). 4) Submits form and notifies supervisor (Rhonda, Mary, Alicia) of need for Autism consultation. IEP Team 1) Determines need & recommends consultation. 2) Completes ASD Core Team Referral Form. 3) Obtains Parental signature on form (required). 4) Notifies supervisor (Rhonda, Mary, Alicia) of need for Autism consultation. ASD Core Team Referral submitted to Autism Consultant * Medical or Educational Diagnosis of Autism • Why use the ASD Core Team Referral Form? • Lists IEP/SIT team expectations and problem statement. • Contains data used for consultation. • Contains vital information about student. • Provides Release of information • TASN requires for reimbursement. Autism Consultant 1) Reviews referral form for completeness. 2) Responds to IEP/SIT team with availability. 3) May Visit and observe for more data. 4) Develops plan to support student and teacher/ Consults with ASD Core Team. See Page 2 for Detailed Screening/Evaluation Process.

Autism Consultant 1) Reviews referral form for completeness. 2) Responds to IEP/SIT team with availability. 3) May Visit and observe for more data. 4) Develops plan to support student and teacher/ Consults with ASD Core Team. ASD Core Team Meets (once a month) to review requests for Autism Screening/ Evaluation. 2 Team members assigned to perform Autism Screening. Parents / SPED/SIT Team notified and consulted for further steps ASD Core Team writes report and sends recommendations to IEP/SIT team. Negative Screen Positive Screen 4 ASD Core Team Members meet with Student and Parents and conduct evaluations , then submit reports to KU-MED. TASN provides materials and oversight for evaluations* Parent submit Release forms, Insurance forms, copy of Medical /Insurance Card * Autism Assessment and parent interview used by KU-MED in diagnosis. ASD Core Team coordinates Tele-Med conference with Parents and KU-MED for formal recommendations. ASD Core Team helps IEP/SIT Team, Student, & Family with KU-MED recommendations.
