  10. Instructions to access the Safety Report database 1. To get to the Safety Report database, go to Valmont Online homepage. From there, you place the cursor on the Resource Center button on the website toolbar, go to Safety in the pull-down menu, then select Safety Report. Select “International Structures Safety Report”. Safety and Environmental Seminar, Charmeil Oct. 2009 Select “Click here to View/Update the International Safety Report” (the only button you can select). Select the link to update the Safety Report with your site’s data. • Select most recently completed quarter and current year from pull-down menus. • Select your facility location from the pull-down menu. • There are two main areas of data to update - Drivers (proactive activities that are believed to reduce risk and improve safety programs) and Results (measures that reflect recent performance in safety). Explanation of the data to be entered into the Safety Report Database Elements of Safety Drivers Total SBOs (Safety By Objectives) for Year – the number of safety activities or program improvements you have chosen to complete for the year (safety inspections, employee training sessions, job safety analyses, etc.). SBO is the name of the site-specific annual safety and health plan.  The site management team (along with the safety coordinator) develops their annual plan and tracks each activity to completion.  The plan shall be reviewed at least every 3 months. Completed SBOs for Quarter - the number of safety objectives/activities completed for the quarter. SBO Plan Reviews – the number of times in the past quarter that the plan has been reviewed by the management team or safety committee Ergonomic Projects – projects that identify ergonomic hazards and then implement controls to eliminate/reduce them. The number of ergonomic projects that are completed for the quarter are entered into the database. Hazard Elimination – the number of hazards eliminated. These should be documented. At a minimum, there should be a list of hazards that were identified and controls that were implemented to eliminate the hazards. Leader Safety Tours – the number of tours performed by site safety leaders.  This will be explained further during orientation. Diagnosis – This is the number of completed investigations performed as a result of an injury, illness, property loss, or other incident.  The goal of a diagnosis/investigation is to identify the root cause and to improve a part of the management system to eliminate the root cause. This could be an improvement in employee training, a written procedure, a hazard assessment, employee discipline, equipment maintenance, etc. Persons in Plan – the number of persons involved in developing and being responsible for activities in the annual SBO Plan. This reflects involvement of site personnel in safety objectives. Safety Performance Reviews – the number of reviews. This reflects accountability that’s expected of employees. Best Practices – the number of best practices from the site safety team. Best practices are collected from companies outside of Valmont. Safety Perception Survey – Score (in %) of the safety perception survey that company employees complete. This survey is performed every year. Last Updated – date and time of last entry into the database.  This is automatically entered by the software. Elements of Safety Results Total Employees – avg. number of employees that worked at facility in the quarter Hours Worked – total number of hours worked by Valmont and temporary employees Days w/o Lost Time Incident – number of days since the last lost time injury/illness. LTIs YTD  -  the number of Lost Time Incidents that have occurred Year-To-Date Lost Work Day Rate – the database calculates this for you, based on the hours worked and the LTIs YTD data. Days Away From Work – the number of days that employees were out of work due to an occupational injury and/or illness. The day of the injury or illness is not counted. Total Injuries OSHA Rec. – the number of injury and/or illness cases that meet the definition of an Occupational Safety and Health Administration (OSHA) recordable case.  OSHA is a U.S. government agency.  For facilities outside the United States, these cases are called Medical Management cases, but the definition is the same.  Hopefully this will help keep the data consistent between facilities. Recordable Rate - the database calculates this for you, based on the hours worked and the Total Injuries OSHA Rec. data # of Restricted or Trans. Cases – the number of work-related injury and/or illness cases where the employee’s job tasks needed to be restricted or the employee needed to be transferred to another job/area due to the injury or illness.  Further definition is available for this. Days of Restricted Work OTJ – the total number of days that employees were restricted or transferred (from cases included in the previous element) No. of First Aid Cases – the number of work related injuries or illnesses that meet the definition of first aid cases – these tend to be less serious than OSHA Recordable/Medical Management Cases. Worker’s Comp Costs – this is the total amount of money (in U.S. dollars) that has been incurred, or reserved to pay for medical and salary costs for work related injuries or illnesses. Property Damage Costs – Cost of time and material spent by maintenance on repairs that were not expected and not the result of fair-wear-tear (normal wear and tear of equipment and/or the facility). Compliance Costs – money/fines levied by outside safety regulatory agencies for internal safety or health violations Awards – the number of safety recognition awards presented during the quarter
