Full Proposal Requirements • When completing your Full Proposal be sure to • include all the information in this online presentation, as it will impact the scoring of HSCI applications and will determine the funding of proposed initiatives. • Please note: All HSCI initiatives must emphasize the reduction/elimination of health disparities and should clearly demonstrate the promotion of health equity. Submissions not adequately impacting health disparities are not likely to receive funding.

Full Proposal Requirements • A detailed Statement of Need • Clear, concise description of the Community Capacity in place to implement the proposed initiative • A detailed Action Plan Narrative and HSCI Strategy Specific Work Plan(s) • A detailed Budget(s)and Justification(s) for requested funds

Full Proposal Requirements • What do I include in the Statement of Need ? • (500 word limit) (15 points) • Include information about what problem(s) the project is addressing • Provide a brief summary of the community need and chronic disease burden with a focus on health disparities • Note: Please do not submit statistics for the state of South Carolina—all data should be specific to your target area/county/neighborhood.

Full Proposal Requirements • What do I include in the Community Capacity section? • (300 word limit) (15 points) • Explain your capacity to achieve the stated HSCI goals and outcomes in the targeted community/among the target population. • Explain your capacity to implement the project activities. Be sure to list key partners and successes/outcomes, as well as any barriers to community partnerships. • Demonstrate clearly your ability to bring together stakeholders that support your proposed activities.

Full Proposal Requirements • What do I include in the Action Plan Narrative? • (50 points) • Provide a detailed description regarding activities necessary to achieve the goals and objectives of each HSCI strategy selected • Provide a detailed description regarding the roles and responsibilities of selected partners as they relate to the proposed initiative

Full Proposal Requirements • What do I need to focus on in the • HSCI Strategy Specific Work Plan ? • (50 points) • Objectives for each strategy • Activities for each objective—make sure activities are necessary to achieving your objectives. They should also be Policy, Systems, and Environmental change based and within HSCI funding parameters • Description of activities for each partner listed in the Full Proposal

  9. HSCI Strategy Specific Work Plan (50 points) Fill out all contact information. Select funding category from the drop-down menu. Community Action applicants need to select a strategy/strategies and submit a separate work plan for each. An objective is a statement describing the results to be achieved and the manner in which these results will be achieved (see slide 12 for specific information on developing SMART objectives ) • Describe your activities and be sure to provide a timeframe • Identify a milestone—something you can accomplish that lets you know that you are on target • Be sure to note any progress you have already made, or that you hope to make according to your timeframe

  10. Full Proposal Requirements In writing the Action Plan Narrative and in filling out Strategy Specific Work Plans, applicants should use SMARTObjectives*: Specific Measurable Achievable Realistic Time-phased *See the next slide for more information.

Full Proposal Requirements • Specificobjectives include who will be targeted and what will be accomplished • Measurableobjectives include how much change is expected with enough specificity that the achievement of the objective can be measured through counting or through documentation of change or completion • Achievableobjectives can be accomplished given existing resources and constraints • Realisticobjectives address the scope of the problem and reasonable programmatic steps • Time-phasedobjectives provide a time line indicating when the objective will be measured or a time by which the objective will be met

Full Proposal Requirements • What do I include in the Budget and Justification? • (20 points) • A strategy specific budget with detailed line item justification of expenditures designed to achieve HSCI goals and objectives • Please note: Applicants need to submit a separate budget worksheet and justification for each HSCI strategy selected. All budget requests should be within HSCI funding parameters outlined by the Centers for Disease Control and Prevention (CDC).

  13. HSCI Budget Worksheet and Justification Make sure to submit a separate budget worksheet for each HSCI strategy. Select a strategy and funding category from the drop down menus. If you are a Community Engagement Grant applicant, you do NOT need to select a strategy. Enter amounts in the appropriate line item box. Be sure to provide a description of expenditures. All expenditures need to be directly related to accomplishing HSCI goals and objectives. Please note: Expenditures may be refused if inflated or if not inherent to the success of the proposed initiative.

  14. 14 What Happens Next? • All applicants submitting a Full Proposal that meets HSCI requirements will receive confirmation of submission. • Applicants will be notified of grant awards no later than March 18, 2013.
