  2. Science Fair Dates • Science Bound Science Fair: Tuesday, January 24 • District Science Fair: Thursday, February 16th, 2012 • State Science Fair: Friday & Saturday, March 30 & 31, 2012 • All local/building fairs should be held prior to February 1st, 2012

  3. Science Fair Project Criteria • Any 6th-8th Grade DMPS student • May work in pairs or individually • Science Bound students are encouraged to enter • Projects will be judged in 3 categories, Earth, Life and Physical Sciences • Projects must have the required paperwork

  4. Required Paperwork • Adult Sponsor Checklist • Student Checklist (1A) • Approval Form (1B) • Research Plan (please note a bibliography with 5 sources is required) • Abstract *Forms 1A, 1B and the Adult Sponsor Checklist will be required for the district fair this year.

  5. Supplemental Paperwork • Qualified Scientist Form (required for projects involving human subjects, vertebrate animals, hazardous biological agents, and DEA-Controlled substances) • Risk Assessment Form (required for any projects using hazardous chemicals, activities or devices) • Human Participants Form & Voluntary Consent (required for any project involving human subjects, including surveys) • Vertebrate Animal Form A or B (required for all projects involving vertebrate animals)

  6. Science Fair Display Boards and Funding • If you need additional display boards for your building, please email Kim with the number needed by October 14. • Please reuse old display boards by covering them with paper or fabric if they are still in good shape. • You may use building science department budget to cover costs associated with your building fair.

  7. Additional Items • We will continue to use the state approved judging form for the district fair (a committee will be formed to look at a more detailed approach) • A district bus will be provided to the state fair again this year, and teacher subs will be paid for by district funds • Consult the State of Iowa Science Fair website for details regarding photographs and other display criteria • http://www.sciencefairofiowa.org
