  1. JOHN J. SASSANO, M.D. Interventional Pain Management

  2. NORTH FLORIDA PAIN SPECIALISTS believe … • You have the right to be believed and respected • You have the right to have your chronic pain diagnosed and treated • You have the right to receive state-of-the-art treatment and technology to manage your pain • You have the right to understand the cause of your pain and what your treatment options are • You have the absolute right to reduce suffering and improve your quality of life

  3. JOHN J. SASSANO, M.D. • Board Certified in Pain Management by The American Board of Anesthesiology • Former Director of Pain Management at The University of Florida • Pain Fellowship, University of Pittsburgh Medical Center • Pain Fellowship, Texas Tech University • 1989 “Inventor of the Year” for his patented “Rapid Venous Infusion System” • Was injured in a ski accident and experienced chronic pain himself … Dr. Sassano DOES understand!

  4. WHICH TYPE OF PAIN IS YOURS? Acute • Acute pain is self-limiting, meaning it goes away and responds to short term medication Chronic • Chronic pain is frequent or constant, does not respond well to traditional therapies, and lasts at least several months • Nociceptive • Caused by stimulation of peripheral nerve fibres (crushing/tearing etc.) and can be either somatic (affecting the ‘outside’ of the body, as in bones/joints/muscles) or visceral (affecting the inner organs thru damage or disease) • Neuropathic • Caused by damage or disease affecting how the Central Nervous System processes information, and can be either central (affecting spine/brain) or peripheral (affecting body structures further away from the spine or brain)

  5. INTERVENTIONAL PAIN MANAGEMENT PHYSICIANS treat CHRONIC PAIN Wikipedia defines Interventional Pain Management as the subspecialty of the physician’s specialty (Anesthesiology for example) that is devoted to the diagnosis and treatment of pain and its related disorders. An Interventional Pain Management Physician utilizes ‘interventional techniques’, and is represented by a variety of pain management organizations including The American Board of Anesthesiology, The American Society of Interventional Pain Physicians, The International Spine Intervention Society, and The Florida Society of Interventional Pain .

  6. NORTH FLORIDA PAIN SPECIALISTS provide INTERVENTIONAL PAIN MANAGEMENT • Spinal Cord Stimulation • Lumbar Epidural Steroid Injection • Lumbar Sympathetic Block • Lumbar Epidurolysis (RACZ) Procedure • Sacroiliac Joint Injection • Medial (Nerve) Branch Block for Facet Joints

  7. NORTH FLORIDA PAIN SPECIALISTS ARE AGGRESSIVE IN THE FIELD OF SPINAL CORD STIMULATION • A Spinal Cord Stimulator is a ‘neuromodulation’ device implanted into the spine during a minimally invasive procedure. It provides pinpoint delivery of small doses of electricity directly to nerve sites. Spinal Cord Stimulation works by delivering electrical impulses that interfere with the way the Central Nervous System processes pain signals. And at the same time the SCS is sending pain inhibiting signals down the spine, it is sending ‘feel-good’ signals back up to the brain, creating a pleasant sensation. Dr. Sassano is a well-respected Interventional Pain Physician, performing SCS trials on a one-week basis. After a successful ‘trial’ of the stimulator, you then together determine whether permanent implantation is the answer to your chronic pain.

  8. SPINAL CORD STIMULATION could be right for you if … • You have Failed Back Surgery Syndrome • You have Radiculopathy (nerve root pain) • You have Neurogenic Pain • You have Arachnoiditis • You have Epidural Fibrosis • You have Peripheral Neuropathy • You have Multiple Sclerosis • You have RSD

  9. SPINAL CORD STIMULATOR INDICATIONS: • You are no longer a candidate for spine surgery • You have tried (and failed) medical, surgical, physical, and psychological therapies • You are free of infection, do not have a pacemaker or implanted defibrillator, and are not currently on blood thinning medication • You are motivated, and understand what will occur during the implantation/trial process

  10. NORTH FLORIDA PAIN SPECIALISTS provide LUMBAR EPIDURAL STEROID INJECTIONS An Epidural Steroid Injection is a long-lasting cortisone injection given into the space surrounding the spinal cord and the nerves (the epidural space). The injection takes only a few moments, and reduces the inflammation and swelling of nerves in the epidural space. In turn, this may reduce pain, tingling, numbness, and the various other symptoms associated with nerve inflammation.

  11. NORTH FLORIDA PAIN SPECIALISTS provide LUMBAR SYMPATHETIC BLOCKS A Lumbar Sympathetic Block refers to an injection of local anesthetic into the nerves of the back located on either side of the spine (sympathetic nerve tissue). The injection is a quick procedure that serves to block the sympathetic nerves. This may then reduce pain and swelling, and positively affect color and sweating changes that might have occurred in the extremities. It may also improve mobility.

  12. NORTH FLORIDA PAIN SPECIALISTS provide LUMBAR EPIDUROLYSIS (RACZ) PROCEDURES The Epidurolysis (RACZ) procedure is used to dissolve some of the scar tissue (adhesions) from around entrapped nerves in the epidural space of the spine. Reducing adhesions enables other medications, like cortisone for example, to more readily reach the affected areas. The procedure involves 1 or 2 injections. Scarring is often caused from bleeding in the epidural space following back surgery, or could simply occur when a disk ruptures. Once scar tissue is dissolved, further injections of painkillers have a greater chance of diminishing symptoms.

  13. NORTH FLORIDA PAIN SPECIALISTS provide SACROILIAC JOINT INJECTIONS This procedure involves an injection of long-lasting cortisone into the sacroiliac (SI) joints, and takes only a few moments. The sacroiliac joints are located in the lower back area. The steroid injected during an SI procedure reduces the inflammation and swelling of tissue in the joint space. In turn, this may reduce the pain and other symptoms caused by irritation of the joints and surrounding structures.

  14. NORTH FLORIDA PAIN SPECIALISTS provide MEDIAL BRANCH BLOCKS for FACET JOINTS The facet joints are located between vertebrae in the spine. They are innervated, or supplied, by nerves called medial branches. These nerves carry pain signals thru the spinal cord to the brain. Blocking, or numbing them, is like ‘cutting the wires’ and disturbing communication of pain to the brain. If pain is due to facet joint disease, such as arthritis, a medial branch block is expected to provide relief from discomfort and stiffness. Once a diagnosis of facet joint disease is confirmed, we use a procedure called “Radio Frequency Lesioning”, which can prevent the conduction of pain information to the brain for several months.
