Western Health is addressing challenges in the implementation of target processes 7.1 and 9.6. A quality improvement steering group has been established to monitor milestones and report to the Best Care Coordinated Care sub-committee. An action plan has been developed following gap analysis in specialist outpatient clinics. Ongoing action plans will address unmet target processes, with a focus on improving referral reporting and triage timeframes, as well as enhancing dictation software to support data reporting needs. Continuous monitoring and reevaluation will drive progress.
