  2. How Can We Prepare for Transition? • Develop a focused plan of study (age 14) • Complete age appropriate transition assessments (not just vocational/pre-vocational assessments) • Develop Measurable Post-Secondary Goals • After age 16, IEP driven by measurable post-secondary goals

Transition Assessments • Employment • Education/Training • Independent Living • Community Participation (No longer just interest or aptitude assessments; must relate to the 4 areas) • No “one size fits all” • Must be related to MPSGs • Interests, strengths, and preferences • Transition Assessment “Tool Kit”

  5. Writing Measurable Post-Secondary Goals • Results-oriented terms (enrolled in, work, live independently) • Descriptors (full-time, part-time) • Think about where student hopes to be one year after high school • Based on “age-appropriate” transition assessments (not defined by OSEP)

MPSGs are Not Food Additives • Training . . . .”After high School, I will enroll full-time in an on-the-job training program” • Education . . . .”I will enroll full-time at a technical college or university” • Employment . . . .”After high school, I will work full-time for a construction company” • Independent Living . . . .”After high school, I will live with my parents at home”

  7. MPSGs Drive IEPs

  8. Transition Service NeedsMirror MPSGs • Develop course of study that will reasonably enable student to meet MSPGS (age 14) • In transition services, include activities, strategies, and services the student will need to reasonably meet his/her MPSGs (age 16)

ASK QUESTIONS • What does the student want to do to earn a living? • Are their goals realistic? • Where will the student learn the skills to become what he/she wants? • How will they pay for training? • What agencies will help with employment, education/training, independent living, community access? • How much can/will they be involved in the community?

  10. Then Ask More Questions. . . . Post-Secondary/Training/Education • What training programs are offered? • What is length and cost of the programs? • What are the entry requirements? • What support services are available? • How/where can financial assistance be obtained? • What is the application procedure? • Is there assistance in job placement after completion?

Adult Service Agencies • What programs/services are offered by the agency? • What types of disabilities do they serve? • How is eligibility determined and what documentation is needed? • What is the cost? • Is there a waiting list? If so, how long? • Who is the contact person? • What are the age requirements? • How long are the agency services provided?
