In Session 3 of the ASaP initiative, Alberta physicians are sharing their progress in understanding and tracking patient health profiles. With 31 primary care organizations expressing interest and 20 actively participating, the session focuses on lessons learned, documented changes, and measured results from case studies. Our goal is to support primary care teams in sustaining improvements and sharing knowledge. Ongoing support includes updates, recommended changes, and innovative simulations to enhance learning and collaboration among improvement facilitators and care providers.

Purpose of Session 3 • Sharing progress and lessons learned across the participants, and drawing out examples of documented changes tested and implemented, including measured results achieved through Case Study approach. • Planning to support primary care teams to continue their improvements in order to sustain the gains and spread the learning to other teams and other priority topics. • Celebrate successes evident through learning and process improvements.
