Outcomes & Achievements • Awaiting discussions from Oxfordshire CCG as they are discussing Outcomes based commissioning. • Historically trialed POS / SKIPP • ANP / CNS review for the whole organisation - portfolio‘s for team • Commissioning income more stable from reporting and reconcilliation.

  3. Challenges • Increased complexity of patients • Increased challenges working within community setting as less district nurses and carers • Role blur between community matrons & specialist nurses • IT challenges • Retention & recruitment to specialist posts, particularly band 7 posts • Commissioning changes across Oxfordshire has seen numerous changes • Little scope for development and expansion in current financial climate

  4. New & Future Plans • Have introduced band 6 posts as increased resource and succession planning • Away days for whole team to support team development and resilience • Better liaison with Oxford Health • Wider Trust strategy for end of life care • Wider Trust and National work on resilience
