  4. Developments – Infrastructure • Service delivery NOT an IT project • Getting buy in from CH(C)Ps • Embedding within clinical teams • Improving reporting and performance management • Restructure of Telehealth Board

  5. Developments – Infrastructure • Developing links with SCT • Looking beyond TeleScot • Learning from DOH Whole System Demonstrator Sites • Gathering Evidence Based data relevant to Lothian • New NHS Lothian eHealth Strategy to include telehealth

Incentivisation • Recruitment to Telehealth vital to progress RCT and the programme • Small amount of LTC monies available to Edinburgh GPs to cover costs of work to identify suitable patients to be referred for inclusion in COPD • Appropriate referrals remunerated • Practices given information regarding process to be followed including referral pathway and patient information • Currently a small amount of patients have been referred via this initiative

  8. Clinical Areas • COPD – Telescot Trial and Non Trial • Heart Failure – service development with some qualitative evaluation via Telescot • Childhood Obesity – planning stage • Hypertension – Telescot Trial • Diabetes – Telescot Trial -planning stage

  9. Equipment • 400 Intel Health Guides available – used with COPD, Heart Failure and Childhood Obesity cohorts • Hypertension trial uses mobile phone technology

  10. Statistics – COPD and Heart Failure • At 03/03/10: • 136 referrals to Telehealth (includes 24 pilot patients)

  11. Statistics - COPD • Majority of COPD Telehealth patients are linked to the RCT or the pilot prior to this • At 03/03/10: • Referred for Telehealth: 81 patients • 59 patients active on system 24 pilot 35 post pilot • 14 Work in progress • 8 withdrawn

  12. Heart Failure • North West Edinburgh and West Lothian service development • Heart Failure Nursing Service involved with monitoring of some patients who are on their caseload • Under ‘umbrella’ of Telescot so some qualitative evaluation will take place with patients

  13. Heart Failure • At 03/03/10 • Referrals to Telehealth: 55 • 32 patients active on system • 16 withdrawn • 7 Work in progress

Childhood Obesity Service • Role of Telehealth to reach out and provide a clinic at home • Service in planning to allow consultation in the home via Televideoconferencing function by Dietetic Staff • Equipment – Health Guide and scales • Aim is to have 30 patients with Telehealth equipment for 3 years (each for I yr)

Overall Challenges and Issues • Staffing – consent, training, monitoring • Standard protocols,procedures, documentation • Consent • Monitoring – different models • Risk assessment • Costs including need for different peripherals e.g. type of probe • Problem resolution e.g. videoconferencing • Qualitative work e.g. organisation of digital story

  16. Monitoring • Different models in place across Lothian within teams • Robust longer term solution required • A number of options being pursued currently

  17. Lessons Learned • Lessons learned log in place • Used for videoconferencing issue • Log will be developed as programme progresses
