Difficult Issues Facing Hospitals Today • A national survey of ED department directors confirms that two thirds reported inadequate on-call specialist coverage. • All Medicare-participating hospitals with emergency departments must provide a medical screening exam, followed by stabilization and further care or transfer, as needed, regardless of the patient’s ability to pay. • Emergency Medical Treatment and Active labor Act (EMTALA) obligations, including ensuring adequate on-call coverage, fall predominately on hospitals, not physicians. • Shortages in neurosurgeons, orthopedic surgeons, obstetrics, ophthalmologists, head and neck surgeons, plastic, and hand surgeons.
